高氧戊己汤对幽门螺旋杆菌感染小鼠胃黏膜保护作用的研究

摘  要:目的通过观察高氧戊己汤对幽门螺旋杆菌(Helicobacter pylori,Hp)感染小鼠体内Hp定植、胃黏膜组织病理学损伤的改善情况以及白细胞介素-6(interleukin-6,IL-6)、白细胞介素-8(interleukin-8,IL-8)表达的影响,分析高氧戊己汤对Hp感染小鼠胃黏膜保护作用的机理。方法将72只SPF级BalB/c小鼠随机分为空白组、模型组、戊己丸组、高氧水组、高氧戊己汤组、西药组。除空白组外,另外5组采用Hp菌液复制Hp感染模型。造模成功后,对应给药治疗2周后处死取胃组织,快速尿素酶实验检测Hp感染情况,ELISA法检测IL-6、IL-8的表达水平,瑞氏-吉姆萨染色法观察胃黏膜Hp的定植情况,HE染色法观察胃黏膜病理学损伤改善情况。结果各治疗组的Hp转阴率明显高于模型组,高氧戊己汤组的转阴率高于戊己丸组和高氧水组;与空白组比较,模型组IL-6、IL-8的表达显著增加(P<0.01),Hp定植评分显著增高(P<0.01),与模型组比较,戊己丸组、高氧水组、高氧戊己汤组、西药组IL-6、IL-8的表达显著减少(P<0.01),Hp定植评分显著降低(P<0.01)。高氧戊己汤组IL-6、IL-8的表达较戊己丸组和高氧水组有降低趋势,高氧戊己汤组Hp定植评分低于戊己丸组、高氧水组(P<0.05)。HE染色结果显示空白组小鼠胃黏膜光滑完整,未见炎症浸润,腺体排列整齐。模型组小鼠胃黏膜充血水肿,表面欠光滑,黏膜层可见淋巴细胞、中性粒细胞等炎症细胞浸润,腺体排列紊乱。戊己丸组、高氧水组、高氧戊己汤组、西药组小鼠胃黏膜炎症程度明显减轻,胃黏膜充血水肿较模型组稍改善,表面光滑,腺体整齐,可见少量炎症细胞浸润。结论高氧戊己汤、戊己丸和高氧水可通过减少Hp在胃黏膜上的定植和减轻Hp感染所致的炎症反应来减少胃黏膜组织病理学损伤,从而有效地保护Hp感染小鼠的胃黏膜,且高氧戊己汤药效高于戊己丸和高氧水单用。Objective By observing the improvement of Helicobacter pylori(Hp)colonization and pathological damage to gastric mucosal tissue in mice infected with Hp,as well as the effects of high oxygen Wuji Decoction on the expressions of interleukin-6(IL-6)and interleukin-8(IL-8),the mechanism of high oxygen Wuji Decoction protecting gastric mucosa in Hp infected mice was analyzed.Methods Seventy-two SPF BalB/c mice were randomly assigned into blank group,model group,Wuji Pills(戊己丸)group,high oxygen water group,high oxygen Wuji Decoction group and western medicine group.Except for the blank group,the other 5 groups used Hp bacterial fluid to replicate the Hp infection model.After successful modeling,the stomach tissue was sacrificed after 2 weeks of drug administration,and the infection of Hp was detected by rapid urease test,the expression levels of IL-6 and IL-8 were detected by ELISA,and the stomach was observed by Wright′s Giemsa staining.The colonization of mucosal Hp was observed,and the improvement of gastric mucosal pathological damage was observed by HE staining.Results The Hp conversion rate of each treatment group was significantly higher than the model group.The conversion rate of the high oxygen Wuji Decoction group was higher than that of the Wuji Pills and the high oxygen groups.Compared with those of the normal group,the expressions of IL-6 and IL-8 in the model group were increased significantly(P<0.01)and the Hp planting evaluation was significantly increased(P<0.01).Compared with those of the model group,the expressions of IL-6 and IL-8 of the Wuji Pills group,the high oxygen water group,the high oxygen Wuji Decoction group and the western medicine group were significantly reduced(P<0.01),and the Hp planting evaluation was significantly reduced(P<0.01).The expressions of IL-6 and IL-8 in high oxygen Wuji Decoction group had a lower trend compared to those in Wuji Pills and high oxygen groups(P<0.05).The HE dyeing results showed that the gastric mucosa of the blank group was smooth and complete,th
