肌骨超声检查在痛风性关节炎患者疾病活动度评估中的应用效果研究

摘　　要：目的分析肌骨超声对评估痛风性关节炎(GA)患者疾病活动度的应用效果。方法选取100例GA患者纳入观察组,另将96例非GA患者纳入对照组。所有患者均行肌骨超声检查。对比两组患者肌骨超声影像表现以及不同疾病活动度GA患者的肌骨超声半定量评分。结果观察组患者共有109处病变关节,对照组患者共有100处病变关节。两组肌骨超声表现为骨化、骨侵蚀、滑膜增生、滑膜炎的占比对比,差异无统计学意义(P>0.05);观察组肌骨超声表现为痛风石、高回声点、平行线占比分别为44.04%、52.29%、39.45%,高于对照组的12.00%、6.00%、8.00%,腱鞘炎占比31.19%低于对照组的69.00%,差异有统计学意义(P<0.05)。高疾病活动度GA患者滑膜增生、滑膜血流信号、关节腔积液、软骨缺损评分分别为(2.79±0.45)、(2.83±0.51)、(2.69±0.46)、(2.75±0.42)分,均高于中疾病活动度GA患者的(2.16±0.31)、(2.20±0.40)、(2.01±0.29)、(2.14±0.31)分以及低疾病活动度GA患者的(1.63±0.22)、(1.85±0.32)、(1.54±0.18)、(1.68±0.27)分,差异有统计学意义(P<0.05)。结论肌骨超声检查在GA患者的诊断中效果显著,能够有效评估其疾病活动度,临床应用价值较高。Objective To analyze the practical effect of musculoskeletal ultrasound in evaluating disease activity in patients with gouty arthritis(GA).Methods 100 GA patients were included in the observation group,and another 96 non-GA patients were included in the control group.All patients underwent musculoskeletal ultrasound.Comparison of the musculoskeletal ultrasound imaging performance between the two groups,as well as the musculoskeletal ultrasound semiquantitative scores of GA patients with different levels of disease activity.Results The observation group had a total of 109 diseased joints,while the control group had 100 diseased joints.There was no statistically significant difference in the proportion of ossification,bone erosion,synovial hyperplasia,and synovitis between the two groups(P>0.05).The proportion of gout stones in the observation group was 44.04%,the proportion of high echo points was 52.29%,and the proportion of parallel lines was 39.45%,which were higher than the control group's 12.00%,6.00%,and 8.00%,while the proportion of tenosynovitis was 31.19%,which was lower than the control group's 69.00%,and the difference was statistically significant(P<0.05).The scores for synovial hyperplasia,synovial blood flow signal,joint cavity effusion,and cartilage defect of GA patients with high disease activity were higher than those of patients with medium and low disease activity GA,and the difference was statistically significant(P<0.05).Conclusion Musculoskeletal ultrasound has a significant effect in the diagnosis of GA patients,which can effectively evaluate their disease activity and has high clinical application value.
