自拟益气养阴化瘀汤治疗气阴两虚兼瘀型冠心病稳定型心绞痛的临床疗效观察

摘　　要：目的观察自拟益气养阴化瘀汤治疗气阴两虚兼瘀型冠心病稳定型心绞痛患者的临床治疗效果。方法60例气阴两虚兼瘀型冠心病稳定型心绞痛患者,随机分为观察组(30例)和对照组(30例)。两组均给予基本对症治疗,在此基础上对照组给予抗血小板聚集、调脂、扩冠、减轻心肌耗氧量治疗,观察组在对照组的基础上给予自拟益气养阴化瘀汤治疗。比较两组患者心电图疗效、心绞痛疗效、硝酸甘油减停率、内皮功能指标[内皮素-1(ET-1)、一氧化氮(NO)]及B型尿钠肽(BNP)水平。结果治疗后,观察组心电图改善总有效率50.00%显著高于对照组的23.33%,差异有统计学意义(P<0.05)。治疗后,观察组心绞痛改善总有效率80.00%显著高于对照组的46.67%,差异有统计学意义(P<0.05)。治疗后,观察组ET-1、NO、BNP水平分别为(67.74±5.14)pg/ml、(69.51±6.23)μmol/L、(71.54±11.42)ng/L,显著优于对照组的(84.41±6.23)pg/ml、(55.41±5.89)μmol/L、(161.28±26.54)ng/L,差异有统计学意义(P<0.05)。观察组硝酸甘油减停率为83.33%(25/30),显著高于对照组的53.33%(16/30),差异有统计学意义(χ2=6.239,P=0.012<0.05)。结论通过自拟益气养阴化瘀汤治疗气阴两虚兼瘀型冠心病稳定型心绞痛患者,可以显著缓解患者胸闷、胸痛等临床症状,减少硝酸甘油用量,改善内皮功能,疗效确切,可以在临床推广使用。Objective To observe the clinical effect of self-made Yiqi Yangyin Huayu decoction on stable angina pectoris of coronary heart disease with deficiency of both qi and yin and blood stasis.Methods 60 patients with stable angina pectoris of coronary heart disease with deficiency of both qi and yin and blood stasis were randomly divided into an observation group(30 cases)and a control group(30 cases).Both groups received basic symptomatic treatment.On this basis,the control group was given anti-platelet aggregation,lipid regulation,coronary dilation,reducing myocardial oxygen consumption therapy,and the observation group was given self-made Yiqi Yangyin Huayu decoction on the basis of the control group.The electrocardiogram efficacy,angina pectoris efficacy,reduction rate of nitroglycerin,endothelial function indexes[endothelin-1(ET-1),nitric oxide(NO)]and B-type natriuretic peptide(BNP)levels were compared between the two groups.Results After treatment,the total effective rate of electrocardiogram improvement was 50.00%in the observation group,which was significantly higher than 23.33%in the control group,and the difference was statistically significant(P<0.05).After treatment,the total effective rate of angina pectoris improvement was 80.00%in the observation group,which was significantly higher than 46.67%in the control group,and the difference was statistically significant(P<0.05).After treatment,the levels of ET-1,NO and BNP in the observation group were(67.74±5.14)pg/ml,(69.51±6.23)μmol/L and(71.54±11.42)ng/L,which were significantly better than(84.41±6.23)pg/ml,(55.41±5.89)μmol/L and(161.28±26.54)ng/L in the control group,and the difference was statistically significant(P<0.05).The reduction rate of nitroglycerin in the observation group was 83.33%(25/30),which was significantly higher than 53.33%(16/30)in the control group,and the difference was statistically significant(χ2=6.239,P=0.012<0.05).Conclusion Self-made Yiqi Yangyin Huayu decoction can significantly relieve chest tightness and chest pain i
