柔筋养精手法联合中药膏膜治疗膝骨关节炎疼痛

摘　　要：目的探讨柔筋养精手法联合中药膏膜治疗膝骨关节炎(KOA)疼痛的临床应用。方法选取2022年3月-2023年3月安徽中医药大学第二附属医院收治的膝骨关节炎患者84例,根据随机数表法分为研究组与对照组,各42例。研究组采用柔筋养精手法联合中药膏膜治疗,对照组口服塞来昔布胶囊,比较2组疼痛情况、关节功能及血清白细胞介素-1β(IL-1β)、C反应蛋白(CRP)、肿瘤坏死因子-α(TNF-α)水平。结果治疗后研究组膝关节内侧点、外侧点、中心点疼痛阈值高于对照组(P<0.05),简版McGill疼痛问卷调查(SF-MPQ)评分低于治疗前及对照组(P<0.05);治疗后研究组膝关节损伤与骨关节炎评分(KOOS)评分高于治疗前及对照组(P<0.05);治疗后研究组血清IL-1β、TNF-α、CRP水平低于对照组(P<0.05);研究组膝骨关节炎患者治疗总有效率(95.24%,40/42)高于对照组(76.19%,30/42)(P<0.05)。结论柔筋养精手法联合中药膏膜治疗可以有效缓解膝骨关节炎患者疼痛症状,降低膝骨关节炎患者关节炎症反应,改善关节功能,提高临床治疗效果。Objective To explore the clinical application of manipulations for"soothing tendons and nourishing essence"combined with ointment membrane of Chinese medicine in the treatment of pain in knee osteoarthritis(KOA).Methods A total of 84 patients with knee osteoarthritis admitted to the Second Affiliated Hospital of Anhui University of Chinese Medicine from March 2022 to March 2023 were selected as the study subjects,and they were divided into a study group and a control group according to a random number table method,with 42 cases in each group.The study group was treated with manipulations for"soothing tendons and nourishing essence"combined with ointment membrane of Chinese medicine,while the control group was treated with celeoxib capsules orally.The pain level,joint function and serum levels of interleukin-1β(IL-1β),C-reactive protein(CRP),tumor necrosis factor-α(TNF-α)were compared between the two groups.Results After treatment,the pain thresholds of the medial,lateral and central points of the knee joint were higher in the study group than those in the control group,and the scores of the short-form McGill pain questionnaire(SF-MPQ)were lower than those before treatment and in the control group(P<0.05).After treatment,the knee injury and osteoarthritis scores(KOOS)were higher in the study group than those in the control group(P<0.05).After treatment,the serum levels of IL-1β,TNF-αand CRP were lower in the study group than those before treatment and in the control group(P<0.05).The effective rate of KOA patients in the study group was higher than that of the control group(P<0.05).Conclusion Manipulations for"soothing tendons and nourishing essence"combined with ointment membrane of Chinese medicine can effectively relieve pain symptoms in KOA patients,reduce joint inflammatory reactions,improve joint function in KOA patients,and improve clinical treatment effect.
