摘　　要：目的:探讨超滤液内回输联机血液透析滤过(HFR)对尿毒症患者的影响。方法:选取2021年1月—2023年12月黔东南州人民医院收治的100例尿毒症患者作为研究对象,随机分为对照组(接受常规血液透析治疗)和观察组(接受HFR治疗),各50例。比较两组临床疗效、钙磷代谢、肾功能和残余肾功能(RRF)、并发症等指标。结果:观察组治疗总有效率高于对照组(P=0.042);治疗后,两组钙、磷离子水平下降,且观察组低于对照组(P<0.05);治疗后,两组尿素氮(BUN)、血清肌酐(Scr)及RRF水平均下降,且观察组BUN、Scr水平低于对照组,RRF水平高于对照组(P<0.05);两组并发症发生率比较,差异无统计学意义(P>0.05)。结论:相较于常规血液透析,HFR在尿毒症患者治疗中应用效果更好,可有效改善钙磷代谢、肾功能指标,保护RRF,且不会增加不良反应发生率。Objective:To investigate the effect of ultrafiltrate on uremia patients by on-line hemodiafiltration(HFR).Methods:A total of one hundred uremia patients admitted to Qiandongnan Prefecture People's Hospital from January 2021 to December 2023 were selected as the study objects,and randomly divided into control group(receiving routine hemodialysis treatment)and observation group(receiving HFR treatment),with fifty cases in each group.The clinical efficacy,calcium and phosphorus metabolism,renal function,residual renal function(RRF)and complications were compared between the two groups.Results:The total effective rate of observation group was higher than that of control group(P=0.042);after treatment,the levels of calcium and phosphorus ions in two groups decreased,and the observation group was lower than the control group(P<0.05);after treatment,the levels of urea nitrogen(BUN),serum creatinine(Scr)and RRF were decreased in both groups,and the BUN and Scr levels in observation group were lower than those in control group,and the RRF levels were higher than those in control group(P<0.05);there was no significant difference in the incidence of complications between the two groups(P>0.05).Conclusion:Compared with conventional hemodialysis,HFR has a better effect in the treatment of uremia patients,which can effectively improve calcium and phosphorus metabolism,renal function indicators,protect RRF,and does not increase the incidence of adverse reactions.
