摘 要:目的:探讨经阴道二维与三维超声技术在子宫内膜息肉(EP)诊断中的应用价值。方法:选取2022年3月—2023年10月商洛市山阳县人民医院收治的疑似EP患者80例为研究对象,分别接受经阴道二维超声与经阴道三维超声检查,以宫腔镜结合病理诊断结果为“金标准”,分析两种检查方法的诊断结果、诊断效能及图像特征。结果:宫腔镜结合病理诊断结果显示,80例疑似EP患者中,阳性60例,阴性20例。经阴道三维超声诊断符合率高于经阴道二维超声,差异有统计学意义(P<0.05)。经阴道三维超声诊断灵敏度、特异度、阳性预测值、阴性预测值高于经阴道二维超声,差异有统计学意义(P<0.05)。经阴道三维超声形态以类圆形为主,且边缘不清晰,与经阴道二维超声比较,差异有统计学意义(P<0.05);两种检查方法在诊断EP部位、直径、回声情况方面,差异无统计学意义(P>0.05)。结论:经阴道二维与三维超声均为EP筛查较为可靠的诊断方法,但经阴道三维超声的诊断效能更高,且图像更加清晰。Objective:To investigate the application value of transvaginal two-dimensional and three-dimensional ultrasound technology in the diagnosis of endometrial polyp(EP).Methods:Eighty patients with suspected EP admitted to Shangluo Shanyang County People's Hospital from March 2022 to October 2023 were selected as the study subjects,and underwent transvaginal two-dimensional ultrasound and transvaginal three-dimensional ultrasound,respectively.The diagnostic results,diagnostic efficacy and image characteristics of the two examination methods were analyzed with hysteroscopy combined with pathological diagnosis as the"gold standard".Results:The diagnostic results of hysteroscopy combined with pathology showed that among eighty patients with suspected EP,sixty cases were positive and twenty cases were negative.The diagnostic compliance rate of transvaginal three-dimensional ultrasound was higher than that of transvaginal two-dimensional ultrasound,and the difference was statistically significant(P<0.05);the diagnostic sensitivity,specificity,positive predictive value,and negative predictive value of transvaginal three-dimensional ultrasound were higher than those of transvaginal two-dimensional ultrasound,and the difference was statistically significant(P<0.05).The morphology of transvaginal three-dimensional ultrasound was predominantly of similar round shape,and the edges were not clear,and the difference was statistically significant compared with that of transvaginal two-dimensional ultrasound(P<0.05).There was no statistically significant difference between the two examination methods in terms of diagnosis of EP sites,diameters,and echogenicity(P>0.05).Conclusion:Transvaginal two-dimensional and three-dimensional ultrasound are both more reliable diagnostic methods for EP screening,but transvaginal three-dimensional ultrasound has higher diagnostic efficacy and clearer images.
