摘  要:目的:评价基于过氧化氢雾化消毒机的集束化管理措施在重症监护室(ICU)多重耐药菌(MDRO)感染预防与控制中的应用效果。方法:将2022年1—12月广州医科大学附属清远医院ICU收治的患者1453例纳入对照组,实施集束化管理,将2023年1—12月医院ICU收治的患者1273例纳入观察组,在对照组基础上使用过氧化氢雾化消毒机对ICU内物体表面进行清洁消毒。比较两组MDRO感染发生率、物体表面消毒合格率和管控行为执行率。结果:观察组MDRO感染发生率低于对照组,差异有统计学意义(P=0.005);观察组物体表面消毒合格率高于对照组,差异有统计学意义(P=0.002);观察组隔离医嘱下达、隔离标识放置、手卫生、单间隔离、医疗用品专用、医疗废物正确处置执行率高于对照组(P<0.05)。结论:基于过氧化氢雾化消毒机的集束化管理可有效降低ICU中MDRO感染发生率,并提高物体表面消毒合格率、管控行为执行率。Objective:To evaluate the application of clustered management measures based on hydrogen peroxide fogging disinfector in the prevention and control of multidrug-resistant bacterial(MDRO)infections in the intensive care unit(ICU).Methods:One thousand four hundred and fifty-three patients admitted to the ICU of Qingyuan Hospital Affiliated to Guangzhou Medical University from January to December 2022 were included in the control group,and cluster management was implemented.One thousand two hundred and seventy-three patients admitted to the ICU of the hospital from January to December 2023 were included in the observation group,and a hydrogen peroxide fogging disinfector was used to clean and disinfect the surfaces of objects in the ICU on the basis of the control group.Compare the incidence rate of MDRO infection,the qualified rate of object surface disinfection and the implementation rate of control behavior in the two groups.Results:The incidence of MDRO infection in the observation group was lower than that in the control group,and the difference was statistically significant(P=0.005);the qualified rate of object surface disinfection in the observation group was higher than that in the control group,and the difference was statistically significant(P=0.002);the implementation rate of isolation medical ordering,isolation marking placement,hand hygiene,single-room isolation,specialization of medical supplies,and correct disposal of medical waste in the observation group was higher than that of the control group(P<0.05).Conclusion:Clustered management based on hydrogen peroxide atomized disinfector can effectively reduce the incidence of MDRO infections in ICU and improve the qualified rate of disinfection of object surfaces and the execution rate of control behaviors.
