基于“因虚生积”理论探讨胃癌前病变的中医病机演变及辨治

摘　　要：胃癌前病变(GPL)是临床常见的消化系统疾病,积极治疗对防止胃癌发生具有重要意义。周斌教授治疗GPL经验丰富,从动态思维出发,提出“因虚生积”理论,认为“脾胃虚弱、气机失调”是其发生根本,“痰瘀滞聚、毒损胃络”是其进展渐阶,“天年衰损、伏络成积”是其癌变的关键;根据阶段病机特点,认为“健脾养胃、调和气机”是其治疗根本,“祛湿化痰,散瘀解毒”可逆转病情进展,“通络消积、固本防变”是截断其癌变的关键。文章基于“因虚生积”理论对GPL全病程的中医病机动态演变及辨治进行系统总结,为临床辨治提供参考。Gastric precancerous lesions(GPL)are common clinical digestive system diseases,and active treatment is of great significance to prevent the occurrence of gastric cancer.Professor ZHOU Bin,with rich experiences in the treatment of GPL,proposes the theory of‘accumulation due to deficiency'from the dynamic thinking.He thinks that the spleen-stomach weakness,disorder of movement of qi'is the fundamental pathogenesis,‘phlegm-blood stasis,damage of stomach collateral by toxic turbidity'promotes disease development,and‘damage of stomach natural year,accumulation of latent pathogen in the stomach collateral'is crucial to gastric tumorigenesis.According to the characteristics of the phased pathogenesis,professor ZHOU Bin argues that‘strengthening the spleen-stomach,regulating qi movement'is the fundamental treatment,‘removing dampness,resolving phlegm,eliminating blood stasis,detoxifying'can reverse disease progression,and‘promoting blood circulation and removing masses,strengthening the foundation and preventing disease from deteriorating'is the key method to stop gastric tumorigenesis.Based on the theory of‘accumulation due to deficiency',this paper systematically summarizes the dynamic evolution of traditional Chinese medicine pathogenesis and treatment of the whole course of GPL,so as to provide reference for clinical treatment.
