朱丹溪辨治肥胖症学术思想探析

摘　　要：肥胖症在古代文献中并未被作为一种疾病记载,其相关论述最早见于《黄帝内经》,即《素问·通评虚实论》云:“甘肥贵人,则高粱之疾也”。继《黄帝内经》后,肥胖症相关内容散见于各类医学文献之中。直至元代朱丹溪始对肥胖症有较为系统而独特的论述。朱丹溪取金元各家之所长,并结合临床经验,首创“肥胖”之名,诊治“先观形色然后察脉问证”,辨证以“气血痰郁”为纲,古方新用,自创新方且善用药对,治疗灵活变通。朱丹溪辨治肥胖症的学术思想不但丰富了后世医家对肥胖症的认识,而且对现代肥胖症及其并发症的临床辨治亦具有极其重要的指导作用。Obesity was not recorded as a disease in the ancient literature,and its related discussion was first seen in the Huangdi Neijing.Suwen·Tongping Xushi Lun said that the rich people got obesity mostly due to long-term diet of fatty,sweet and thick food.Following the Huangdi Neijing,obesity-related contents were scattered in various medical literatures.It was not until ZHU Danxi,Yuan dynasty,began to have a more systematic and unique discussion of obesity.ZHU Danxi took the strengths of the Jin and Yuan schools and combined them with clinical experience to create the name'obesity'.He emphasized the importance of'looking at the appearance first and then checking the pulse and asking for symptoms'in the diagnosis and treatment,and the'qi,blood,phlegm and stagnation'was the platform for identification.In the treatment,he used the ancient prescriptions with a new applications,and made prescription for self-innovation.He also was good use of the medicinal pair,and flexible and adaptable.ZHU Danxi's academic thoughts on the identification and treatment of obesity not only enriched the understanding of obesity among later generations of doctors,but also had an extremely important guiding role in the clinical identification and treatment of modernobesityand its complications.
