大株红景天注射液联合沙库巴曲缬沙坦钠片治疗冠心病合并心力衰竭疗效研究

摘　　要：目的观察大株红景天注射液联合沙库巴曲缬沙坦钠片治疗冠心病合并心力衰竭(CHDWHF)的疗效。方法将我院112例CHDWHF患者随机分为对照组与治疗组各56例。对照组采用沙库巴曲缬沙坦钠片治疗治疗组采用大株红景天注射液联合沙库巴曲缬沙坦钠片治疗。比较两组总有效率、中医症状评分、左室射血分数(LVEF)、左室舒张末内径(LVEDD)、左室收缩末内径(LVESD)、N端前体脑钠肽(NTproBNP)、6 min的步行距离(6MWD)、血细胞比容、血小板黏附性。随访12周比较心力衰竭(HF)恶化再入院、严重心血管不良事件发生情况。结果治疗后,治疗组总有效率高于对照组(P<0.05);两组中医症状评分、LVESD、LVEDD、NT-proBNP、血细胞比容、血小板黏附性均低于治疗前(P<0.05)治疗组上述指标均低于对照组(P<0.05);两组LVEF、6 MWD高于治疗前(P<0.05)治疗组LVEF、6 MWD高于对照组(P<0.05);治疗组HF恶化再入院、严重心血管不良事件发生情况低于对照组(P<0.05)。结论针对CHDWHF采用大株红景天注射液联合沙库巴曲缬沙坦钠片治疗,能够改善中医症状和心室重构,提高心功能,改善血液动力学,提升疗效,降低HF恶化再入院、严重心血管不良事件发生情况。Objective To observe the efficacy of Dazhu Hongjingtian Injection combined with sacubitril valsartan sodium tablets in treatment of coronary heart disease complicated with heart failure(CHDWHF).Methods A total of 112 patients with CHDWHF were randomly divided into control group and treatment group,with 56 cases in each group.Control group was treated with sacubitril-valsartan sodium tablets,and treatment group was treated with Dazhu Hongjingtian Injection on the basis of control group.The total effective rate,TCM symptom score,left ventricular ejection fraction(LVEF),left ventricular enddiastolic diameter(LVEDD),left ventricular end-systolic diameter(LVESD),N-terminal procursor brain natriuretic peptide(NT-proBNP),6-min walking distance(6 MWD),hematocrit,and platelet adhesion of the two groups were compared.The readmission rate and the serious cardiovascular adverse events were compared with HF exacerbation at 12 weeks of follow-up.Results After treatment,the total effective rate of treatment group was higher than that of control group(P<0.05);the TCM symptom scores,LVESD,LVEDD,NT-proBNP,hematocrit and platelet adhesion in both groups were lower than those before treatment(P<0.05),and were lower in treatment group than those in control group(P<0.05);the LVEF and 6MWD in both groups were higher than those before treatment(P<0.05),and were higher in treatment group than those in control group(P<0.05);the readmis sion rate of HF deterioration and the incidence of serious cardiovascular adverse events in treatment group were lower than those in control group(P<0.05).Conclusion Dazhu Hongjingtian Injection combined with sacubitril-valsartan sodium tablets can improve TCM symptoms and ventricular remodeling,improve cardiac function,improve hemodynamics,enhance efficacy,and reduce the readmission rate of HF deterioration and the incidence of serious cardiovascular adverse events in treatment of CHDWHF.
