解毒消痈止痛方联合甲基泼尼松龙琥珀酸钠对腹型IgA血管炎患儿外周血血小板平均体积变化的影响

摘　　要：目的观察解毒消痈止痛方联合甲基泼尼松龙琥珀酸钠对腹型IgA血管炎患儿外周血血小板平均体积(MPV)变化的影响。方法将我院60例腹型IgA血管炎患儿随机分为研究组与对照组各30例。对照组给予甲泼尼龙琥珀酸钠治疗研究组给予解毒消痈止痛方联合甲基泼尼松龙琥珀酸钠治疗。比较两组患儿临床疗效,腹痛缓解、大便隐血转阴、皮疹消失时间,中医证候主症、次症积分,治疗前后免疫球蛋白G(IgG)、IgM、白细胞介素-6(IL-6)、白细胞介素-8(IL-8)、T淋巴细胞CD4^(+)、CD8^(+)、CD4^(+)/CD8^(+)、免疫球蛋白A(IgA)、补体C3、MPV、血小板计数(PLT)。结果治疗后,研究组总有效率高于对照组(P<0.05);研究组腹痛缓解、大便隐血转阴、皮疹消失时间均短于对照组(P<0.05);两组IgG、CD4^(+)、CD4^(+)/CD8^(+)、补体C3、MPV含量均高于治疗前(P<0.05),研究组上述指标均高于对照组(P<0.05);两组中医证候主症、次症积分、IgM、IL-6、IL-8、CD8^(+)、IgA、PLT含量均低于治疗前(P<0.05),研究组上述指标均低于对照组(P<0.05)。结论解毒消痈止痛方联合甲基泼尼松龙琥珀酸钠治疗腹型IgA血管炎患儿疗效较优,可降低中医证候积分,促进体液免疫和细胞免疫恢复正常调节IL-6、IL-8水平提高补体C3水平调节血小板参数。Objective To observe the effect of Jiedu Xiaoyong Zhitong Prescription combined with methylprednisolone sodium succinate on the changes of peripheral platelet volume(MPV)in children with abdominal IgA vasculitis.Methods A total of 60 children with abdominal IgA vasculitis were randomly divided into study group and control group,with 30 cases in each group.Control group was treated with methylprednisolone sodium succinate,and study group was treated with Jiedu Xiaoyong Zhitong Prescription on the basis of control group.The efficacy of the two groups was compared,including the relief of abdominal pain,the conversion of fecal occult blood to negative,the time of rash disappearance,the main and secondary symptoms of TCM syndrome,and the scores of immunoglobulin G(IgG),IgM,interleukin-6(IL-6),IL-8,T lymphocytes CD4^(+),CD8^(+),CD4^(+)/CD8^(+),immunoglobulin A(IgA),complement C3,mean platelet volume(MPV)and platelet count(PLT)before and after treatment.Results After treatment,the total effective rate of study group was higher than that of control group(P<0.05);compared with control group,the reduction time of abdominal pain,fecal occult blood turned negative,and rash disappearance in study group were shorter(P<0.05);the contents of IgG,CD4^(+),CD4^(+)/CD8^(+),complement C3 and MPV in both groups were higher than those before treatment,while the main and secondary symptoms of TCM syndrome,the points of IgM,IL-6,IL-8,CD8^(+),IgA and PLT were lower(P<0.05),and the indexes above were all superior in study group to those in control group(P<0.05).Conclusion Jiedu Xiaoyong Zhitong Prescription combined with methylprednisolone has excellent efficacy in treatment of children with abdominal IgA vasculitis,which can reduce the TCM syndrome score,promote the normalization of humoral immunity and cellular immunity,regulate the levels of IL-6 and IL-8,increase the level of complement C3,and regulate platelet parameters.
