阳和通脉丸联合前列地尔对糖尿病下肢动脉硬化闭塞症患者肢体冷感、静息痛的影响

摘　　要：目的观察阳和通脉丸联合前列地尔对糖尿病下肢动脉硬化闭塞症(DAOOLL)患者肢体冷感、静息痛的影响。方法将我院70例DAOOLL患者随机分为对照组与治疗组各35例。对照组给予前列地尔静滴,治疗组在对照组的治疗基础上口服阳和通脉丸。比较两组疗效、足背动脉血流量、踝肱指数、趾肱指数、管襻、流态及襻周积分、襻顶血管直径,症状评分,检测C反应蛋白(CRP)、肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)、血凝素样氧化低密度脂蛋白受体-1(LOX-1),比较不良反应发生率。结果治疗后,治疗组总有效率高于对照组(P<0.05);两组足背动脉血流量、踝肱指数、趾肱指数、襻顶血管直径高于治疗前,治疗组足背动脉血流量、踝肱指数、趾肱指数、襻顶血管直径高于对照组(P<0.05);两组下肢血管襻、流态及襻周积分低于治疗前,治疗组下肢血管襻、流态及襻周积分低于对照组(P<0.05);两组症状评分低于治疗前,治疗组症状评分低于对照组(P<0.05);两组CRP、TNF-α、IL-6、LOX-1水平低于治疗目前,治疗组CRP、TNF-α、IL-6、LOX-1水平低于对照组(P<0.05);两组不良反应发生率比较无统计学意义(P>0.05)。结论阳和通脉丸联合前列地尔可显著改善DAOOLL患者肢体冷感、静息痛等症状降低炎症反应程度减轻血管壁损伤,改善微循环和血流动力学。Objective To observe the effect of Yanghe Tongmai Pills and alprostadil on cold sensation and resting pain in diabetes patients with lower extremity arteriosclerosis obliterans(D AOOLL).Methods 70 DAOOLL patients were randomly divided into control group and treatment group,with 35 cases each.Control group received intravenous infusion of prostaglandin,while treatment group received Yanghe Tongmai Pills on the basis of control group.The efficacy,dorsal foot artery,blood flow,ankle brachial indexes,symptom scores,C-reactive protein(CRP)and tumor necrosis factor-α(TNF-α),interleukin-6(IL-6),hemagglutinin like oxidized low-density lipoprotein receptor-1(LOX-1),vascular loop,flow state and periloop integral,parietal vessel diameter,and the incidence of adverse reactions of the two groups were compared.Results After treatment,the total effective rate of treatment group was higher than that of control group(P<0.05);and were lower in treatment group than those in control group(P<0.05);the blood flow of the dorsalis pedis artery,ankle brachial index,toe brachial index,and diameter of the vessel at the top of the loop in both groups were higher than before treatment,and were higher in treatment group than those in control group(P<0.05);the lower limb vascular loop,flow state,and loop circumference scores of both groups were lower than before treatment,and were lower in treatment group than those in control group(P<0.05);the symptom scores of both groups were lower than before treatment,and were lower in treatment group than control group(P<0.05);the levels of CRP and TNF-α,IL-6 and LOX-1 in both groups were lower than before treatment,and were lower in treatment group than those in control group(P<0.05);there was no statistically significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion Yanghe Tongmai Pill combined with prostaglandin significantly improves symptoms such as limb coldness and resting pain in DAOOLL patients,reduces the degree of inflammatory response,reduces va
