一例心脏受累的抗线粒体抗体阳性的炎性肌病

摘　　要：抗线粒体抗体(AMA)阳性的炎性肌病是一种以慢性进行性呼吸肌无力、肌肉萎缩和心脏受累为特征的非典型炎性肌病。心脏受累可表现为心律失常、心肌病和心肌炎等,早期发现和治疗这种肌病可以预防严重的心血管不良事件,从而改善心功能。本文报道了一例以心房扑动为首发心脏表现的AMA阳性的炎性肌病一例,以提高临床医师对此类炎性肌病的认识。Anti-mitochondrial antibody(AMA)-positive inflammatory myopathy is an atypical inflammatory myopathy characterized by chronic progressive respiratory muscle weakness,muscle atrophy,and cardiac involvement.Arrhythmias,cardiomyopathy,and myocarditis have been reported as cardiac manifestations.Early identification and treatment of this myopathy can prevent serious cardiovascular adverse events and thereby improve cardiac function.We report a case of AMA-positive inflammatory myopathy with atrial flutter as the first cardiac manifestation,in order to improve clinicians′understanding of this inflammatory myopathy.
