摘  要:目的探讨外周血25-羟基维生素D[25(OH)D]、人β防御素-2(HBD-2)及胰岛素样生长因子1(IGF-1)水平与痤疮患者疾病严重程度的相关性。方法选取128例寻常痤疮患者纳入痤疮组,另选取128例健康体检者纳入对照组。检测2组外周血25(OH)D、HBD-2及IGF-1水平。分析外周血25(OH)D、HBD-2及IGF-1水平与痤疮患者疾病严重程度的关系。分析痤疮患者外周血25(OH)D、HBD-2和IGF-1水平间的关系。采用受试者工作特征(ROC)曲线评估25(OH)D、HBD-2和IGF-1水平对重度痤疮的诊断价值。结果痤疮组外周血25(OH)D水平低于对照组,IGF-1水平高于对照组,差异有统计学意义(P<0.05);2组HBD-2水平比较,差异无统计学意义(P>0.05)。痤疮组中,随着患者疾病严重程度加重,其外周血25(OH)D水平依次降低,IGF-1水平依次升高,差异有统计学意义(P<0.05)。痤疮患者外周血25(OH)D水平与IGF-1水平、疾病严重程度呈负相关(r=-0.509、-0.586,P<0.05),IGF-1水平与疾病严重程度呈正相关(r=0.547,P<0.05),HBD-2水平与25(OH)D、IGF-1水平及疾病严重程度均无关(P>0.05)。外周血25(OH)D水平、IGF-1水平评估重度痤疮的敏感度分别为94.12%、61.26%,特异度分别为70.59%、77.48%,二者联合评估重度痤疮的敏感度为90.91%,特异度为71.43%。结论痤疮患者外周血25(OH)D水平降低,IGF-1水平升高,且其与疾病严重程度有关。25(OH)D、IGF-1水平联合评估重度痤疮的效能较好。Objective To investigate the correlations of levels of 25-hydroxyvitamin D[25(OH)D],humanβ-defencin-2(HBD-2)and insulin-like growth factor 1(IGF-1)in peripheral blood with disease severity of acne patients.Methods A total of 128 patients with acne vulgaris were included in the acne group and 128 healthy subjects were included in the control group.The levels of 25(OH)D,HBD-2 and IGF-1 in peripheral blood of two groups were detected.The relationships between the levels of 25(OH)D,HBD-2,IGF-1 in peripheral blood and the disease severity of acne patients were analyzed.The relationships between 25(OH)D,HBD-2 and IGF-1 levels in peripheral blood of acne patients were analyzed.The diagnostic value of 25(OH)D,HBD-2 and IGF-1 levels in severe acne was evaluated using receiver operating characteristic(ROC)curves.Results The level of 25(OH)D in peripheral blood of the acne group was significantly lower than that of the control group,and the level of IGF-1 was significantly higher than that of the control group(P<0.05).There was no significant difference in HBD-2 levels between the two groups(P>0.05).In the acne group,with the aggravation of disease severity,the level of 25(OH)D in peripheral blood was significantly decreased,and the level of IGF-1 was significantly increased(P<0.05).The level of 25(OH)D in peripheral blood of acne patients was negatively correlated with the level of IGF-1 and the severity of the disease(r=-0.509,-0.586,P<0.05),and the level of IGF-1 was positively correlated with the severity of the disease(r=0.547,P<0.05).HBD-2 level was not correlated with 25(OH)D,IGF-1 level and disease severity(P>0.05).The sensitivity of peripheral blood 25(OH)D level and IGF-1 level in evaluating severity acne was 94.12%,61.26%,respectively,and the specificity was 70.59%,77.48%,respectively.The sensitivity and specificity of combined evaluation of severe acne were 90.91%and 71.43%,respectively.Conclusion The level of 25(OH)D is decreased and the level of IGF-1 is increased in patients with acne,which is related to the
