摘 要:目的分析探讨“杯指数体系”糖尿病饮食指导的临床效果。方法将2022年7月至2024年2月我院收治的2型糖尿病患者100例为研究对象,依据随机数字表法将入选者分为两组,对照组和观察组各50例。对照组行常规营养指导,观察组则实施“杯指数体系”糖尿病饮食指导,评价患者营养状态(前白蛋白、白蛋白、总蛋白)、血糖控制情况[空腹血糖(FBG)、餐后2 h血糖(2 hPBG)、糖化血红蛋白(HbAlc)]、免疫功能(CD3^(+)、CD4^(+)、CD8^(+)、CD4^(+)/CD8^(+))及自我效能感情况。结果观察组血糖控制达标时间短于对照组,同时其波动控制达标率、3个月HbA1c达标率均高于对照组(P<0.05);营养状态数值(前白蛋白、白蛋白、总蛋白)比较可见观察组高于对照组(P<0.05);干预后,观察组CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+)均高于对照组,CD8^(+)低于对照组(P<0.05);观察组血糖水平(FBG、2 hPBG、HbAlc)均低于对照组(均P<0.05);组间患者接受干预前自我效能感无明显数据差异(P>0.05),护理后观察组得分高于对照组(P<0.05)。结论临床为糖尿病患者实施“杯指数体系”糖尿病饮食指导对患者血糖控制及免疫功能的改善有积极作用,同时可促进营养状态及自我效能感的改善。Objective To analyze and discuss the clinical effect of"cup index system"in diabetes diet guidance.Methods A total of 100 patients with type 2 diabetes who were admitted to our hospital from July 2022 to February 2024 were selected as the research objects.According to the random number method,the participants were divided into two groups,the control group and the observation group,with 50 cases in each group.The control group received conventional nutritional guidance,and the observation group received"cup index system"dietary guidance for diabetes.Nutritional status(prealbumin,albumin,total protein),blood glucose control[fasting blood glucose(FBG),2-hour postprandial blood glucose(2 hPBG),Hemoglobin A1c(HbAlc)]immune function(CD3^(+),CD4^(+),CD8^(+),CD4^(+)/CD8^(+))and self-efficacy were evaluated.Results The blood glucose compliance time of observation group was shorter than that of control group,and the fluctuation control compliance rate and 3-month HbA1c compliance rate were higher than those of control group(P<0.05).The nutritional status(prealbumin,albumin,total protein)of observation group was higher than that of control group(P<0.05).After intervention,CD3^(+),CD4^(+),CD4^(+)/CD8^(+)in observation group were higher than those in control group,and CD8^(+)was lower than those in control group(P<0.05).Blood glucose levels(FBG,2 hPBG,HbAlc)in observation group were lower than those in control group(all P<0.05).There was no significant difference in self-efficacy before intervention(P>0.05),and the score of observation group was higher than that of control group after nursing(P<0.05).Conclusions Clinical implementation of"cup index system for diabetes patients"diabetes diet guidance has a positive effect on the improvement of blood sugar control and immune function of patients,and can promote the improvement of nutritional status and self-efficacy.
