摘 要:目的探讨尿沉渣白细胞(WBC)及细菌定量计数检验在尿路感染诊断中的效能。方法回顾性研究2023年1月至2023年12月在福建省漳州市医院诊治的250例疑似尿路感染患者的临床资料。入院后均进行尿沉渣WBC和细菌计量检验,并以尿细菌培养结果为金标准,分析尿沉渣WBC与细菌定量计数单一及联合检验的效果,并绘制受试工作者特征(ROC)曲线分析不同检验的诊断效能。结果通过检测,尿细菌培养的阳性率为83.20%,以大肠埃希菌、链球菌、葡萄球菌多见。二者联合的阳性率高于单一检测(P<0.05),但与尿细菌培养的阳性率基本一致(P>0.05)。以尿细菌培养结果为金标准,尿沉渣WBC联合细菌定量计数诊断尿路感染的灵敏度、特异度及准确度分别为94.23%、76.19%、91.20%,均高于单一检测(P<0.05)。通过ROC曲线分析,尿沉渣WBC联合细菌定量计数的AUC为0.943(95%CI 0.914~3.119),最佳阳性判断值WBC为88.75 U/μl、细菌总数2650个/μl。结论尿沉渣WBC及细菌定量计数检验联合用于尿路感染诊断有着较高检出率,且有良好诊断效能。Objective To explore the efficacy of urine sediment white blood cell(WBC)and bacterial quantitative count tests in the diagnosis of urinary tract infections.Methods The clinical data of 250 patients with suspected urinary tract infection diagnosed and treated in Zhangzhou City Hospital,Fujian Province from January 2023 to December 2023 were retrospectively studied.Urinary sediment WBC and bacteriometric tests were performed after admission to the hospital,and the results of urine bacterial culture were used as the gold standard to analyse the effect of single and combined tests of urinary sediment WBC and bacterial quantitative counts,and the diagnostic efficacy of different tests was analysed by plotting the characteristics of the subject workers(ROC)curves.Results By testing,the positive rate of urine bacterial culture was 83.20%,with Escherichia coli,Streptococcus,and Staphylococcus being the most common.The positive rate of the combination of the two was higher than that of the single test(P<0.05),but it was basically the same as that of the urine bacterial culture(P>0.05).Using the results of urine bacterial culture as the gold standard,the sensitivity,specificity and accuracy of urine sediment WBC combined with quantitative bacterial counting in diagnosing urinary tract infection were 94.23%,76.19%and 91.20%,respectively,which were higher than that of the single test(P<0.05).By ROC curve analysis,the AUC of urine sediment WBC combined with quantitative bacterial counting was 0.943(95%CI 0.914-3.119),and the optimal positive judgement values were 88.75 U/μl for WBC and 2650 bacteria/μl for total bacteria.Conclusions The combination of urine sediment WBC and bacterial quantitative count test has a high detection rate and good diagnostic efficacy for the diagnosis of urinary tract infections.
