摘  要:目的探究呼出气一氧化氮(FeNO)检测对糖尿病患者慢性咳嗽的价值。方法收集2022年1月至2023年1月我院糖尿病慢性咳嗽患者260例作为观察组,同期260例单纯糖尿病患者作为对照组,检测两组患者FeNO水平及肺功能指标,比较不同病因慢性咳嗽患者FeNO水平、肺功能指标,分析FeNO与肺通气功能的相关性。结果观察组患者FeNO水平高于对照组,第一秒用力呼气容积(FEV1)、第一秒用力呼气量占用力肺活量比值(FEV1/FVC)水平低于对照组(均P<0.05)。不同病因慢性咳嗽患者FEV1、FEV1/FVC水平差异无统计学意义(P>0.05),咳嗽变异型哮喘(CVA)组FeNO水平高于上气道咳嗽综合征(UACS)组、慢性支气管炎组与感染后咳嗽组(P<0.05);FeNO与慢性咳嗽患者肺通气指标基本不相关(P>0.05)。结论FeNO检测能够辅助糖尿病患者慢性咳嗽的诊断,有效识别病因,为临床诊疗提供参考。Objective To explore the value of exhaled nitric oxide(FeNO)in chronic cough of diabetes patients.Methods We collected 260 patients with diabetic chronic cough from January 2022 to January 2023 as the observation group,and 260 patients with simple diabetes mellitus as the control group during the same period,tested the FeNO level and lung function indexes of the two groups,compared the FeNO level and lung function indexes of the patients with chronic cough of different aetiologies,and analysed the correlation between FeNO and lung ventilation function.Results Patients in the observation group had higher FeNO levels than those in the control group,and lower levels of forced expiratory volume in one second(FEV1)and forced expiratory volume in one second as a ratio of forced vital capacity(FEV1/FVC)than those in the control group(all P<0.05).There was no statistically significant difference in FEV1 and FEV1/FVC levels in patients with chronic cough of different etiologies(P>0.05),and FeNO levels were higher in the cough variant asthma(CVA)group than in the upper airway cough syndrome(UACS)group,chronic bronchitis group and post-infectious cough group(P<0.05).FeNO was largely uncorrelated with lung ventilation indices in patients with chronic cough(P>0.05).Conclusions FeNO detection can assist the diagnosis of chronic cough in patients with diabetes,effectively identify the causes,and provide reference for clinical diagnosis and treatment.
