单次大剂量氨甲蝶呤联合米非司酮治疗异位妊娠患者的效果

摘　　要：目的:观察单次大剂量氨甲蝶呤联合米非司酮治疗异位妊娠患者的效果。方法:回顾性分析2021年1月至2023年12月该院收治的70例异位妊娠患者的临床资料,按氨甲蝶呤注射液给药方案不同分为观察组和对照组各35例。观察组采用单次大剂量氨甲蝶呤联合米非司酮治疗,对照组采用多次小剂量氨甲蝶呤联合米非司酮治疗,比较两组β-人绒毛膜促性腺激素(β-HCG)转阴时间、包块消失时间、月经复潮时间、实验室指标[血管内皮生长因子(VEGF)、抑制素(INH-A)、妊娠相关血浆蛋白-A(PAPP-A)]水平、性激素指标(β-HCG、雌二醇、孕酮)水平和不良反应发生率。结果:观察组β-HCG转阴时间、包块消失时间均短于对照组,差异有统计学意义(P<0.05);治疗后,两组血清VEGF、INH-A、PAPP-A等实验室指标和β-HCG、雌二醇、孕酮等性激素指标水平均低于治疗前,且观察组低于对照组,差异有统计学意义(P<0.05);两组月经复潮时间和不良反应发生率比较,差异均无统计学意义(P>0.05)。结论:单次大剂量氨甲蝶呤联合米非司酮治疗异位妊娠患者可缩短β-HCG转阴时间和包块消失时间,降低实验室指标和性激素指标水平,效果优于多次小剂量氨甲蝶呤联合米非司酮治疗。Objective:To observe effects of single high-dose Methotrexate combined with Mifepristone in treatment of patients with ectopic pregnancy.Methods:The clinical data of 70 patients with ectopic pregnancy admitted to this hospital from January 2021 to December 2023 were retrospectively analyzed.According to the different administration methods of Methotrexate injection,they were divided into observation group and control group,35 cases in each.The observation group was treated with single high-dose Methotrexate combined with Mifepristone,while the control group was treated with multiple low-dose Methotrexate combined with Mifepristone.The negative conversion time of human chorionic gonadotropin(β-HCG),the mass disappearance time,the menstrual recovery time,the laboratory indexes[vascular endothelial growth factor(VEGF),inhibin(INH-A),pregnancy-associated plasma protein-A(PAPP-A)]levels,the sex hormone indexes(β-HCG,estradiol,progesterone)levels,and the incidence of adverse reactions were compared between the two groups.Results:The negative conversion time ofβ-HCG and the mass disappearance time in the observation group were shorter than those in the control group,and the differences were statistically significant(P<0.05).After the treatment,the levels of serum VEGF,INH-A,PAPP-A and other laboratory indexes and sex hormones such asβ-HCG,estradiol and progesterone in the two groups were lower than those before the treatment,those in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).However,there were no significant differences in the menstrual recovery time and the incidence of adverse reactions between the two groups(P>0.05).Conclusions:Single high-dose Methotrexate combined with Mifepristone in the treatment of ectopic pregnancy can shorten the negative conversion time ofβ-HCG and the mass disappearance,and reduce the levels of laboratory indexes and sex hormone indexes.Moreover,it is superior to multiple low-dose Methotrexate combined wit
