摘　　要：目的了解山东省威海市生殖道沙眼衣原体感染的流行病学特征,为制定防控措施提供依据。方法通过中国疾病预防控制信息系统收集2012—2021年威海市报告的生殖道沙眼衣原体感染病例资料,采用描述性流行病学方法对其进行分析。结果2012—2021年威海市累计报告生殖道沙眼衣原体感染病例4385例,年均报告发病率为15.55/10万,报告发病率总体呈先上升后下降的趋势(χ_(趋势(2012—2016年))^(2)=134.31、χ_(趋势(2016—2020年))^(2)=76.46,P均<0.05),2021年报告发病率大幅度上升,达20.44/10万。各年度报告发病率差异有统计学意义(χ^(2)=230.45,P<0.05);男性1764例(40.23%),女性2621例(59.22%),年均发病率分别为12.37/10万、18.81/10万;各年度男、女性报告发病率的差异均具有统计学意义(χ^(2)=148.17、248.42,P均<0.05)。20~<40岁组共3419例(77.97%),25~<30岁年龄组的年均发病率最高(64.53/10万),15~<20岁年龄组报告发病率年均增长率最高(23.83%)。报告病例职业以家务待业、商业服务和农民为主,共4048例(92.32%);其中农民报告病例数年均增长率最高(27.96%)。环翠区报告病例数和年均发病率最高(3378例,38.27/10万),发病率年均增长率以乳山市最高(30.41%)。结论2012—2020年,威海市生殖道沙眼衣原体感染报告发病率有所波动,2021年显著增加。青少年、待业人群、农民为针对性筛查、管理和健康宣教的重点人群。Objective To explore the epidemiologic characteristics of genital chlamydia trachomatis(CT)infection in Weihai City and to inform the development of prevention and control strategies.Methods Data on genital CT infection cases reported in Weihai City from 2012 to 2021 were retrieved from the“China Disease Prevention and Control Information System”.Descriptive epidemiological analysis was conducted on the collected data.Results A total of 4385 cases of genital CT infection were reported in Weihai City over the ten-year period,with an average annual incidence rate of 15.55 per 100000 population.The incidence initially increased and then decreased(χ_( trend(2012-2016))^(2)=134.31,χ_(trend(2016-2020))^(2) =76.46,P均<0.05),with a notable surge in 2021(20.44 per 100000).Significant interannual variability was observed(χ^(2)=230.45,P<0.05).Males accounted for 1764 cases(40.23%)and females for 2621(59.22%),with average annual incidence rates of 12.37 and 18.81 per 100000,respectively.Both genders showed significant annual variabilities in reported incidence(χ^(2)=148.17,248.42,P<0.05 for all).There majority of cases,3419(77.97%),were in the 20-<40 age group,with the highest average annual incidence in the 25-<30 age group(64.53per 100000).The 15-<20 age group experienced the steepest annual increase in incidence(23.83%).The most commonly affected occupations were housework,commercial services,and farmers,with a total of 4048 cases(92.32%),with farmers showing the highest annual increase(27.96%).Huancui District reported the highest number of cases and average annual incidence(3378 cases,38.27 per 100000),while Rushan City had the highest annual growth rate(30.41%).Conclusions The reported incidence of genital CT infection in Weihai City fluctuated over the study period,with a significant increase in 2021.Adolescents,unemployed,and farmers are identified as key populations for targeted screening,management,and health education initiatives.
