耳内镜下鼓室成形术加祛腐生肌散外用治疗慢性中耳炎合并鼓膜穿孔的效果

摘　　要：目的探讨耳内镜下鼓室成形术加祛腐生肌散外用治疗慢性中耳炎合并鼓膜穿孔的效果。方法选取2019年7月至2021年7月我院收治的100例慢性中耳炎合并鼓膜穿孔患者为研究对象,根据随机数字表法将其分为对照组和研究组,各50例。对照组接受耳内镜下鼓室成形术+常规药物治疗,研究组在对照组基础上加祛腐生肌散外用治疗。比较两组的治疗效果。结果治疗后,研究组的气导听阈值、气骨导差及耳鸣残疾评估量表(THI)评分低于对照组,差异具有统计学意义(P<0.05);研究组的鼓膜穿孔愈合率高于对照组,差异具有统计学意义(P<0.05)。治疗3个月后,研究组的白三烯D4(LTD4)、基质金属蛋白酶-2(MMP-2)及基质金属蛋白酶-9(MMP-9)水平均低于对照组,差异具有统计学意义(P<0.05)。治疗3个月后,研究组的中耳炎症状、主观听力水平、社会心理影响及医疗资源使用评分低于对照组,差异具有统计学意义(P<0.05)。研究组的并发症总发生率低于对照组,差异具有统计学意义(P<0.05)。结论耳内镜下鼓室成形术加祛腐生肌散外用治疗慢性中耳炎合并鼓膜穿孔能够获得理想的效果,有助于改善患者的生活质量。Objective To explore the effect of endoscopic tympanoplasty combined with Qufu Shengji powder for external use in the treatment of chronic otitis media complicated with tympanic membrane perforation.Methods A total of 100 patients with chronic otitis media complicated with tympanic membrane perforation admitted in our hospital from July 2019 to July 2021 were selected as the research objects.According to the random number table method,the patients were divided into control group and study group,with 50 cases in each group.The control group received endoscopic tympanoplasty+conventional drug treatment,and the study group was treated with Qufu Shengji powder for external use on the basis of the control group.The therapeutic effects of the two groups were compared.Results After treatment,the air conduction threshold,air-bone conduction difference and Tinnitus Handicap Inventory(THI)score in the study group were lower than those in the control group,and the differences were statistically significant(P<0.05);the healing rate of tympanic membrane perforation in the study group was higher than that in the control group,and the difference was statistically significant(P<0.05).Three months after treatment,the levels of leukotriene D4(LTD4),matrix metalloproteinase-2(MMP-2)and matrix metalloproteinase-9(MMP-9)in the study group were lower than those in the control group,and the differences were statistically significant(P<0.05).Three months after treatment,the scores of otitis media symptoms,subjective hearing level,social psychological impact and medical resource use in the study group were lower than those in the control group,and the differences were statistically significant(P<0.05).The total incidence of complications in the study group was lower than that in the control group,and the difference was statistically significant(P<0.05).Conclusion Endoscopic tympanoplasty combined with Qufu Shengji powder for external use in the treatment of chronic otitis media complicated with tympanic membrane perforation can achieve i
