精准扶贫助力农民跨越“健康贫困”陷阱:理论分析与实证检验

摘　　要：贫病相连的恶性循环是制约农民全面发展的重要因素。从健康贫困陷阱的形成路径与表现形式出发,阐释精准扶贫政策健康减贫的逻辑机理。基于中国家庭追踪调查数据构建以收入困难和医疗费用负担为导向的多维健康贫困指标,对农村居民健康贫困情况进行测度,并通过模糊断点回归设计评估精准扶贫政策的多维健康贫困治理效应及其内在机制。结果表明,精准扶贫政策能帮助贫困人口跨越健康贫困陷阱,有效降低贫困人口的多维健康贫困深度和发生率,且政策效应随时间的推移逐渐增强。进一步研究发现,脱贫攻坚时期精准扶贫政策主要通过增加收入、减轻医疗负担、改善营养状况的方式实现健康减贫。研究对常态化健康扶贫政策的优化和调整具有一定的参考价值。The vicious circle of poverty and disease is a major constraint to the overall devel-opment of farmers.Based on the formation paths and manifestations of the health poverty trap,we construct multidimensional health poverty indicators oriented towards income poverty and medical cost burden and elaborate the logical mechanism of health poverty reduction under the targeted poverty alleviation(TPA)policy.Empirically,this study measures health poverty among rural residents in China using data from the China Family Panel Studies and evaluates the health poverty governance effect of TPA through fuzzy regression discontinuity design.The empirical results show that TPA can help the poor escape the health poverty trap and effectively reduce the depth and incidence of multidimensional health poverty,with the effect of the policy gradually increasing over time.Further research shows that the TPA achieved health poverty reduction mainly by increasing income,reducing medical burden,and improving nutritional status.The study offers valuable insights for the optimization and adjustment of normalized health poverty governance policies.
