摘  要:维生素D是一种维持人体正常生理功能的微量营养素及激素,其通过多种效应激活维生素D受体(VDR)发挥全身及局部作用。维生素D活性形式、VDR基因多态性与肾移植术后各项并发症的发生密切相关,除了参与调控肾移植受者钙磷代谢及心血管疾病外,维生素D及受体还参与调控肾移植受者术后骨异常、肌肉减少症、糖尿病、排斥反应及感染的发生,在肾移植领域发挥着不可替代的作用。本文就维生素D及受体在肾移植术后相关并发症中的作用进行综述,探讨维生素D及其类似物作为肾移植术后辅助用药、VDR作为治疗靶点在肾移植受者并发症预防及治疗中的作用。Vitamin D is a kind of micronutrient and hormone to maintain the normal physiological function of the human body,which activates VDR through various effects to play systemic and local roles.Vitamin D active forms and VDR gene polymorphisms are closely related to the occurrence of complications after kidney transplantation.In addition to regulating calcium and phosphorus metabolism and cardiovascular diseases in kidney transplantation recipients,vitamin D and its receptors also play an irreplaceable role in regulating the occurrence of bone abnormalities,sarcopenia,diabetes,rejection and infection in kidney transplantation recipients.This article reviews the role of vitamin D and receptors in the complications after kidney transplantation,and discusses the role of vitamin D and its analogues as adjunct agents after kidney transplantation and vitamin D receptors as therapeutic targets in the prevention and treatment of complications in kidney transplantation recipients.
