针刺联合甲硫酸新斯的明注射液对产后尿潴留患者排尿情况、膀胱残余尿量及盆底肌张力的影响

摘　　要：目的观察针刺联合甲硫酸新斯的明注射液对产后尿潴留(PUR)患者排尿情况、膀胱残余尿量及盆底肌张力的影响。方法将88例PUR患者按照随机数字表法分为2组,对照组44例予甲硫酸新斯的明注射液足三里穴位注射治疗,治疗组44例予针刺联合甲硫酸新斯的明注射液足三里穴位注射治疗,2组均连续治疗2天。比较2组疗效;比较2组治疗后排尿情况;比较2组治疗前后膀胱残余尿量、尿道疼痛评分、盆底肌张力变化。结果治疗组总有效率97.73%(43/44),对照组总有效率81.82%(36/44),治疗组疗效优于对照组(P<0.05)。治疗组治疗后首次排尿时间较对照组缩短(P<0.05),首次尿量较对照组增多(P<0.05)。2组治疗后膀胱残余尿量均较本组治疗前减少(P<0.05),且治疗组减少更明显(P<0.05)。2组治疗后尿道疼痛评分均较本组治疗前降低(P<0.05),且治疗组降低更明显(P<0.05)。2组治疗后阴道静息压、阴道收缩压、阴道收缩持续时间均较本组治疗前升高(P<0.05),且治疗组升高更明显(P<0.05)。结论针刺联合甲硫酸新斯的明注射液足三里穴位注射治疗PUR,可降低患者膀胱残余尿量及尿道疼痛评分,改善盆底肌张力,有助于患者膀胱功能恢复。Objective To explore the impacts of acupuncture combined with neostigmine on urination,bladder residual urine volume and pelvic floor muscle tension in patients with postpartum urinary retention(PUR).Methods A total of 88 PUR patients were randomly assigned in a 1∶1 ratio to neostigmine(control group)or acupuncture combined with neostigmine(study group)for 2 days.The clinical efficacy,urination,bladder residual urine volume,urethral pain score and pelvic floor muscle tension of the two groups were compared.Results The total effective rate of the study group was obviously increased compared with the control group(97.73%[43/44]vs 81.82%[36/44],P<0.05).After treatment,the first urination time in the study group was obviously shorter than that of the control group,but the first urine volume was obviously more(P<0.05).After treatment,significantly decreased bladder residual urine volume and urethral pain score,and increased vaginal resting pressure,vaginal systolic pressure and vaginal contraction duration were detected in both groups(all P<0.05),and the improvements in the study group were more obvious relative to the control group(all P<0.05).Conclusion Acupuncture combined with neostigmine is effective in the treatment of PUR patients,which can obviously reduce the bladder residual urine volume and urethral pain score,improve the pelvic floor muscle tension.It is helpful to the recovery of bladder function in patients.
