雷火灸对终末期肾病维持性血液透析后疲乏的干预效果

摘　　要：目的研究雷火灸对终末期肾病维持性血液透析后疲乏的干预效果。方法将南通市中医院2020年1月至2023年1月收治的90例终末期肾病维持性血液透析患者按随机数字表法分为对照组及研究组,每组45例。对照组予常规干预,研究组在对照组基础上加用雷火灸,持续干预1个月。比较2组干预前后中医症状评分、多维疲劳量表(MFI-20)评分、Tilburg衰弱量表(TFI)评分、C反应蛋白(CRP)水平、匹兹堡睡眠质量指数量表(PSQI)评分。结果2组干预后各项中医症状评分、MFI-20各项评分、TFI评分、PSQI评分及CRP水平均较本组干预前降低(P<0.05),且研究组均低于对照组(P<0.05)。结论基于补气益肾养血原则的雷火灸能够有效改善终末期肾病维持性血液透析患者的中医症状、疲乏状况及衰弱状况,同时还能改善患者的微炎症状态,提高患者睡眠质量。Objective To study the intervention effect of thunder-fire moxibustion for fatigue in patients with end-stage renal disease(ESRD)underwent maintenance hemodialysis(MHD).Methods Ninety patients with ESRD underwent MHD in Nantong Hospital of Traditional Chinese Medicine from January 2020 to January 2023 were randomly assigned into study group(n=45)and control group(n=45).All patients were managed by routine intervention,and those in study group were additionally given thunder-fire moxibustion.A 1-month intervention was performed to compare traditional Chinese medicine(TCM)symptom score,Multidimensional Fatigue Inventory-20(MFI-20)score,Tilburg Frailty Indicator scale(TFI),C-reactive protein(CRP),Pittsburgh Sleep Quality Index(PSQI)score.Results After intervention,the scores of TCM symptom,MFI-20,TFI,PSQI,as well as the levels of CRP in both groups were significantly decreased(P<0.05),which were significantly decreased in the study group than in the control group(P<0.05).Conclusion The thunder-fire moxibustion based on the theory of Tonifying qi and kidney nourishing blood can effectively improve TCM symptoms,fatigue,weakness,micro-inflammation status and sleep quality in patients with ESRD underwent MHD.
