摘　　要：目的:探究1470 nm激光半导体激光前列腺剜除术(DiLEP)对良性前列腺增生患者围术期指标及国际前列腺症评分(IPSS)的影响。方法:选取新密市中医院72例良性前列腺增生患者为研究对象,以随机数字表法分为A组、B组,各36例,分别实施经尿道前列腺电切术和1470 nm激光DiLEP术治疗,比较两组围术期指标、前列腺特异性抗原(PSA)水平、尿动力学指标、IPSS评分、勃起功能及并发症发生率。结果:B组手术时间、并发症发生率与A组相近,差异无统计学意义(P>0.05);B组血钠下降水平较A组低,膀胱冲洗时间、留置导尿管时间较A组短(P<0.05);术后7 d时B组PSA水平较A组高,术后3个月B组最大尿流(Qmax)水平、国际勃起功能指数-5(IIEF-5)评分较A组高,残余尿量(PVR)水平、IPSS评分较A组低,差异有统计学意义(P<0.05)。结论:对良性前列腺增生患者实施1470 nm激光DiLEP手术治疗,与应用经尿道前列腺电切术相比,可维持其血钠稳定,缩短术后康复时间,减轻前列腺损伤,改善其排尿、勃起功能及前列腺功能,且安全性良好。Objective:To explore the effects of 1470 nm Diode laser enucleation of the prostate(DiLEP)on perioperative indexes and International Prostatic disease score(IPSS)in patients with benign prostatic hyperplasia.Methods:A total Xinmi Hospital of Traditional Chinese Medicine of 72 patients with benign prostatic hyperplasia were selected as the study objects and divided into group A and group B by random number table method,36 cases in each group.Tranthral resection of prostate and 1470 nm laser DiLEP were performed respectively.Perioperative indexes,prostate-specific antigen(PSA)level,urodynamic indexes,IPSS score,erectile function and complication rate were compared between the two groups.Results:The operation time and complication rate of group B were similar to those of group A,with no statistical significance(P>0.05).The decrease level of blood sodium in group B was lower than that in group A,and the time of bladder irrigation and catheter indwelling was shorter than that in group A(P>0.05).The PSA level of group B was higher than that of group A at 7 days after surgery,the maximum urinary flow(Qmax)level and the International Index of Erectile Function-5(IIEF-5)score of group B were higher than that of group A at 3 months after surgery,and the residual urinary volume(PVR)level and IPSS score of group B were lower than that of group A,with statistical significance(P<0.05).Conclusion:Compared with transurethral resection of the prostate,the treatment of 1470 nm laser DiLEP for patients with benign prostatic hyperplasia can maintain stable blood sodium,shorten postoperative rehabilitation time,reduce prostate injury,improve urination,erectile function and prostate function,with good safety.
