摘 要:目的:探究手指再植中使用连续缝合与间断缝合技术进行的指神经和指动脉修复的比较。方法:选取2020年8月至2023年8月南阳文和骨科医院收治的手指再植患者108例为研究对象行前瞻性研究,按术中动脉修复方式不同分为A组54例与B组54例,A组使用连续缝合技术治疗,B组使用间断缝合技术治疗,比较两组指神经和指动脉修复效果,并分析术后6个月手指再植修复失败的影响因素。结果:两组再植成功率、Quick DASH评分比较差异无统计学意义(P>0.05)。二元Logistic回归分析显示,长期吸烟史、离断程度(完全)、静脉修复(0~1处)、血管痉挛均是手指再植修复失败的危险因素(P<0.05)。结论:手指再植中使用连续缝合与间断缝合技术进行的指神经和指动脉修复效果均具有一定效果,长期吸烟史、离断程度(完全)、静脉修复(0~1处)、血管痉挛均是手指再植修复失败的危险因素。Objective:Exploring the comparison of finger nerve and artery repair using continuous suture and intermittent suture techniques in finger replantation.Methods:A prospective study was conducted on 108 patients with finger replantation admitted to Nanyang Wenhe Orthopedic Hospital from August 2020 to August 2023,They were divided into Group A with 54 cases and Group B with 54 cases according to different intraoperative arterial repair methods.Group A was treated with continuous suture technology,while Group B was treated with intermittent suture technology.The repair effects of finger nerves and arteries in the two groups were compared,and the influencing factors of finger replantation repair failure at 6 months after surgery were analyzed.Results:There was no statistically significant difference in the success rate of replantation and Quick DASH score between the two groups(P>0.05).Binary logistic regression analysis showed that long-term smoking history,degree of disconnection(complete),venous repair(0~1 locations),and vascular spasm were all risk factors for finger replantation repair failure(P<0.05).Conclusion:The use of continuous suture and intermittent suture techniques in finger replantation has certain effects on the repair of finger nerves and arteries.Long term smoking history,degree of disconnection(complete),vein repair(0~1 locations),and vascular spasm are all risk factors for finger replantation repair failure.
