摘  要:目的研究微RNA-27(miR-27)、脂蛋白(a)[LP(a)]、糖化白蛋白(GA)与糖尿病血管并发症(DVC)的相关性。方法前瞻性选取2020年2月至2022年2月于沧州市人民医院治疗的2型糖尿病(T2DM)患者100例,按照并发症情况分别纳入无糖尿病并发症组(n=25)、糖尿病下肢血管病变组(n=35)及糖尿病肾病组(n=40)。纳入本院体检中心同时期健康体检者20名为对照组。比较4组临床资料;血清总胆固醇、甘油三酯、HDL-C、LDL-C、FPG、HbA1c等常规生化指标;血清LP(a)、GA、miR-27等指标水平;导致DVC发生的影响因素;分析DVC患者临床资料及总胆固醇、甘油三酯、高密度脂蛋白(HDL-C)、低密度脂蛋白(LDL-C)、空腹血糖、糖化血红蛋白(HbA1c)、LP(a)、GA、miR-27等指标与DVC的相关性。结果无糖尿病并发症组、糖尿病下肢血管病变组、糖尿病肾病组的体重指数、总胆固醇、甘油三酯、HDL-C、LDL-C、FPG、HbA1c、LP(a)、GA、miR-27均高于对照组,差异均有统计学意义(P<0.05)。糖尿病下肢血管病变组、糖尿病肾病组的T2DM病程、总胆固醇、甘油三酯、HDL-C、LDL-C、FPG、HbA1c、LP(a)、GA、miR-27均高于无糖尿病并发症组,糖尿病肾病组的T2DM病程、总胆固醇、甘油三酯、HDL-C、LDL-C、FPG、HbA1c、LP(a)、GA、miR-27均高于糖尿病下肢血管病变组,差异均有统计学意义(P<0.05)。T2DM病程、LP(a)、GA、miR-27为导致DVC发生的独立危险因素(OR=1.723,95%CI:1.559~2.318;OR=1.978,95%CI:1.658~2.574;OR=1.916,95%CI:1.620~2.539;OR=1.660,95%CI:1.549~2.214;P<0.05)。经Pearson相关分析,T2DM病程及LP(a)、GA、miR-27水平与DVC正相关(r=0.865、0.773、0.806、0.957,P<0.05)。结论T2DM病程、LP(a)、GA、miR-27水平与DVC正相关。Objective To investigate the correlation between microRNA-27(miR-27),lipoprotein(a)[LP(a)],glycated albumin(GA)and diabetic vascular complication(DVC).Methods A total of 100 patients with type 2 diabetes mellitus(T2DM)who were treated in Cangzhou People's Hospital from February 2020 to February 2022 were prospectively selected and included in without diabetic complications group(n=25),diabetic lower extremity vascular disease group(n=35)and diabetic nephropathy group(n=40)according to the different complications.Twenty healthy subjects in physical examination center of the same hospital at the same time were included as control group.The clinical data of the four groups were compared.Serum total cholesterol,triglyceride,HDL-C,LDL-C,FPG,HbA1c and other conventional biochemical indexes;Serum LP(a),GA,miR-27 and other indexes;The influencing factors leading to DVC;Clinical data of DVC patients and the correlation between total cholesterol,triglyceride,high-density lipoprotein(HDL-C),low-density lipoprotein(LDL-C),fasting blood glucose(FPG),glycated hemoglobin(HbA1c),LP(a),GA,miR-27 and other indexes with DVC were analyzed.Results The body mass index,total cholesterol,triglyceride,HDL-C,LDL-C,FPG,HbA1 c,LP(a),GA and miR-27 in the non-diabetic complication group,diabetic lower extremity vascular disease group and diabetic nephropathy group were higher than those in the control group,and the differences were statistically significant(P<0.05).The course of T2DM,total cholesterol,triglyceride,HDL-C,LDL-C,FPG,HbA1c,LP(a),GA and miR-27 in diabetic lower extremity vascular disease group and diabetic nephropathy group were higher than those in non-diabetic complication group,the course of T2DM,total cholesterol,triglyceride,HDL-C,LDL-C,FPG,HbA1c,LP(a),GA and miR-27 in diabetic nephropathy group were higher than those in diabetic lower extremity vascular disease group,and the differences were statistically significant(P<0.05).The course of T2DM,LP(a),GA and miR-27 were independent risk factors for DVC(OR=1.723,95%CI:1.559-2.3
