两种手术方式对老年不稳定型股骨转子间骨折疗效比较  被引量:1

摘  要:目的比较双极长柄人工股骨头置换与经皮加压钢板对老年不稳定型股骨转子间骨折患者的疗效。方法选择2019年1月至2021年10月就诊的老年不稳定型股骨转子间骨折患者100例,并使用随机数字表法分为A组和B组,各50例。A组行双极长柄人工股骨头置换,B组行经皮加压钢板。比较两组手术情况、术后各时间点的髋关节功能、术后并发症。结果与B组相比,A组手术时间、术中出血量、术后首次下地时间、术后并发症发生率明显较低(P<0.05);术后1、3、6、12个月,两组患者Harris评分明显提高(P<0.05);术后1、3、6个月,A组Harris评分明显高于B组(P<0.05)。结论与经皮加压钢板相比,双极长柄人工股骨头置换改善老年不稳定型股骨转子间骨折患者手术情况及髋关节功能、降低术后并发症的作用较佳。Objective To compare the effect of bipolar long-handle femoral head replacement and percutaneous compression plate in elderly patients with unstable intertrochanteric fractures.Methods 100 elderly patients with unstable intertrochanteric fracture of femur admitted from January 2019 to October 2021 were included and divided into group A and Group B using random number table method,with 50 patients in each group.The group A was given bipolar long-shank prosthetic femoral head replacement and the group B was given percutaneous compression plate.The operation conditions,Harris functional at each postoperative time point and postoperative complications were compared between the two groups.Results Compared with the group B,the operation time,intraoperative blood loss,time of first postoperative landing and incidence of postoperative complications in the group A were significantly lower than those in the group B(P<0.05).At 1,3,6 and 12 months after the operation,the Harris score in the two groups was significantly increased(P<0.05).At 1,3 and 6 months after operation,the Harris score in the group A was significantly higher than that in the group B(P<0.05).Conclusion Compared with percutaneous compression plate,bipolar long-shank femoral head replacement has a better effect on improving the operative status and hip function in elderly patients with unstable intertrochanteric fractures and reducing postoperative complications.
