摘 要:目的分析流感样病例标本中的病毒载量对流感病毒分离效果的影响,从而提高流感病毒分离工作的质量和效率。方法2023年1-10月,在区哨点医院和周边区县收集320例核酸阳性流感样病例标本,采用吸附法和直接接种法接种于狗肾传代细胞中,比较流感病毒分离效率;再以实时荧光定量反转录聚合酶链式反应(RT-qPCR)中的Ct值为界,划分为3组进行对比;对相关因素进行分析。结果不同性别流感病毒细胞分离结果差异无统计学意义(直接接种法:χ^(2)=0.676,P>0.05;吸附法:χ^(2)=1.359,P>0.05)。采用直接接种法分离出的流感阳性毒株数明显高于吸附法,差异有统计学意义(χ^(2)=20.516,P<0.05)。Ct值越低,流感病毒分离阳性率越高,不同Ct值分离阳性率差异有统计学意义(直接接种法:χ^(2)=35.594,P<0.05;吸附法:χ^(2)=15.496,P<0.05)。不同接种方法中,甲型流感亚型H3N2和新甲H1N1分离的流感病毒阳性率差异无统计学意义(直接接种法:χ^(2)=1.563,P>0.05;吸附法:χ^(2)=1.214,P>0.05)。结论在流感病毒细胞分离实验中,采用直接接种法,选取流感病毒载量高(Ct值<30)的标本可以有效提高流感病毒分离阳性率。Objective To analyze the effect of viral load in influenza⁃like illness on the isolation efficiency of influenza viruses,and improve the quality and efficiency of influenza virus isolation work.Methods From Jan.to Oct.2023,320 nucleic acid⁃positive influenza⁃like illness specimens were collected from the district sentinel hospitals and surrounding districts and counties,inoculated into Mardin⁃Darby canine kidney cells using adsorption and direct inoculation methods to compare the efficiency of influenza virus isolation.The specimens were then divided into three groups based on the threshold cycle in reverse transcription⁃quantitative polymerase chain reaction(RT⁃qPCR)for comparison,and relevant factors were analyzed.Results There was no significant difference in the effect of gender on the results of influenza virus isolation(direct inoculation:χ^(2)=0.676,P>0.05;adsorption:χ^(2)=1.359,P>0.05).The number of influenza virus strains isolated by direct inoculation method was significantly higher than that by adsorption method,the difference was statistically significant(χ^(2)=20.516,P<0.05).The lower the CT value,the higher the positive rate of influenza virus isolation.The differences in the separated positive rates for different Ct values was statistically significant(direct inoculation:χ^(2)=35.594,P<0.05;adsorption:χ^(2)=15.496,P<0.05).Among the different inoculation methods,there was no significant difference in influenza virus positive rate isolated from Influenza A subtype H3N2 and H1N1(direct inoculation:χ^(2)=1.563,P>0.05;adsorption:χ^(2)=1.214,P>0.05).Conclusion In influenza virus cell isolation experiments,using the direct inoculation method and selecting specimens with high influenza virus load(Ct value<30)could effectively increase the positive rate of influenza virus isolation.
