贯叶连翘提取物对自身免疫性心肌炎大鼠心肌损伤的保护作用及心肌胶原代谢的影响

摘　　要：目的探究贯叶连翘提取物(Hypericum perforatum extract,HPE)对实验性自身免疫性心肌炎(experimental autoimmune myocarditis,EAM)大鼠心肌损伤的保护作用及心肌胶原代谢的影响。方法6周龄Lewis大鼠采用猪心肌肌球蛋白乳浊液后足垫皮下注射建立EAM模型,将EAM模型大鼠随机分为模型组和HPE低、中、高剂量组各10只,各组大鼠采用灌胃给药方式每次对应给予生理盐水及20、50、100 mg/kg HPE灌胃,每天2次,共28 d,另设空白对照组按照同疗程给予生理盐水。观察大鼠临床表现,测定心脏重量(HW)、体重(BW),计算BW/HW;HE染色后光学显微镜下观察大鼠心脏病理学变化;经超声心动图检测心脏参数左室舒张末内径(left ventricular end-diastolic diameter,LVEDD)、左室收缩末内径(left ventricular end-systolic diameter,LVESD)、左室短轴缩短率(left ventricular fraction shortening,LVFS)、左室射血分数(left ventricular ejection fraction,LVEF);检测大鼠血清心肌损伤指标[肌酸激酶同工酶MB(CK-MB)、心肌钙蛋白T(cTnT)]、胶原代谢指标(Ⅰ型胶原蛋白、Ⅲ型胶原蛋白)水平,蛋白质印迹测定心肌肿瘤坏死因子-α(TNF-α)、转化生长因子(TGF-β1)蛋白水平。结果对比空白对照组,EAM模型大鼠LVEDD、LVESD、HW/BW、CK-MB、cTnT、Ⅰ型胶原蛋白、Ⅲ型胶原蛋白、TNF-α和TGF-β1蛋白水平均升高,LVFS、LVEF水平均降低,心肌组织病理学评分增加,差异有统计学意义(P<0.05);HPE给药后,HPE低、中、高剂量组LVEDD、LVESD、HW/BW、CK-MB、cTnT、Ⅰ型胶原蛋白、Ⅲ型胶原蛋白、TNF-α和TGF-β1蛋白水平均降低,心肌组织病理学评分减少,LVFS、LVEF水平均升高,差异有统计学意义(P<0.05)。结论中药贯叶连翘提取物能改善EAM模型大鼠心肌损伤,可能与其抑制心肌组织炎症反应、减少胶原蛋白沉积有关。Objective To explore the protective effect of Hypericum perforatum extract(HPE)on myocardial injury in rats with autoimmune myocarditis and its influence on myocardial collagen metabolism.Methods Six-week-old Lewis rats were enrolled as the research subjects.The experimental autoimmune myocarditis(EAM)rat model was established by subcutaneous injection of porcine cardiac myosin emulsion into the hind foot pad.The EAM model rats were randomly classified into 4 groups:model group,and low-dose,middle-dose and high-dose HPE groups,with 10 rats in each group.The rats in each group were given intragastric administration of normal saline,20 mg/kg HPE,50 mg/kg HPE or 100 mg/kg HPE,with twice a day for 28 days.The blank control group was additionally set and was given normal saline according to the same course of treatment.The clinical manifestations of rats were observed,and the heart weight(HW)and body weight(BW)were measured,and BW/HW ratio was calculated.The pathological changes of heart were observed under optical microscope after HE staining.Cardiac parameters such as left ventricular end-diastolic diameter(LVEDD),left ventricular end-systolic diameter(LVESD),left ventricular fractional shortening(LVFS)and left ventricular ejection fraction(LVEF)were measured by echocardiography.The levels of serum myocardial injury indicators[creatine kinase isoenzyme MB(CK-MB),cardiac troponin T(cTnT)]and collagen metabolism indicators(type I collagen,typeⅢcollagen)were detected,and tumor necrosis factor-alpha(TNF-α)and transforming growth factor-β1(TGF-β1)protein level was determined by Western blotting.Results The values of LVEDD,LVESD,HW/BW,and the expression levels of CK-MB,cTnT,type I collagen,typeⅢcollagen,TNF-αprotein and TGF-β1 protein in the EAM model rats were increased,the pathological score of myocardial tissues were enhanced,while the values of LVFS and LVEF were decreased,when compared with those in the blank control group(P<0.05).After HPE administration,the values of LVEDD,LVESD,HW/BW,and the expression le
