关节镜手术治疗前踝撞击综合征的疗效分析

摘　　要：目的探讨关节镜手术治疗前踝撞击综合征的疗效。方法武汉大学人民医院2016年4月~2021年10月收治的前踝撞击综合征病人42例,均行关节镜手术,术后对病人行14~26个月随访,采用视觉模拟评分法(visual analogue scale,VAS)评价疼痛,采用美国足与踝关节协会(American Orthopaedic Foot and Ankle Society,AOFAS)踝与后足功能评分评价疗效及踝关节功能情况,比较术前及术后的变化。结果所有病人伤口均Ⅰ期愈合,无感染,无神经、血管、肌腱损伤。术前AOFAS评分为(45.52±6.64)分,术后1个月、术后3个月及术后14个月分别为(76.33±6.52)分、(92.86±3.48)分、(94.95±3.68)分。术前VAS评分为(7.95±0.70)分,术后1个月、术后3个月及术后14个月分别为(3.86±0.65)分、(1.83±0.79)分、(1.24±0.96)分。术后1个月、3个月及14个月AOFAS评分和VAS评分均较术前有显著改善,差异有统计学意义(P<0.05),术后3个月和14个月与术后1个月比较有显著改善,差异有统计学意义(P<0.05)。术后14个月较术后3个月有改善,但差异无统计学意义(P>0.05)。结论关节镜手术治疗踝关节前方撞击综合征,创伤小,恢复快,疗效好,可对合并的韧带和骨软骨损伤有效治疗,中期随访效果稳定。Objective To investigate the effect of arthroscopic surgery on anterior ankle impingement syndrome.Method Arthroscopic surgery was performed on 42 patients with anterior ankle impingement syndrome admitted to the People’s Hospital of Wuhan University from April 2016 to October 2021.The patients were followed up for 14 to 26 months.The visual analogue scale(VAS)was used to evaluate pain,and the American Orthopaedic Foot and Ankle Society,AOFAS(Ankle and hind foot function scores)were used to evaluate the therapeutic effect and ankle joint function,and to compare the preoperative and postoperative changes Results The wounds of all patients were healed in stageⅠwithout infection,nerve,blood vessel or tendon injury.The AOFAS score was(45.52±6.64)before surgery,(76.33±6.52),(92.86±3.48)and(94.95±3.68)at 1 month,3 months and 14 months after surgery,respectively.The VAS score was(7.95±0.70)before surgery,(3.86±0.65),(1.83±0.79)and(1.24±0.96)at 1 month,3 months and 14 months after surgery,respectively.AOFAS scores and VAS scores at 1 month,3 months and 14 months after operation were significantly improved compared with those before operation,and the difference was statistically significant(P<0.05),there was significant improvement at 3 and 14 months after surgery compared with 1 month after surgery,and the difference was statistically significant(P<0.05),14 months after surgery was better than 3 months after surgery,but the difference was not statistically significant(P>0.05).Conclusion Arthroscopic surgery in the treatment of anterior ankle impingement syndrome,less trauma,quick recovery,good effect,can be effective in the treatment of ligamentous and osteochondral injury,mid-term follow-up effect is stable.
