性类固醇激素在盆底功能障碍性疾病的应用

摘　　要：盆底功能障碍性疾病(pelvic floor dysfunction,PFD)目前尚缺全球公认的安全有效的药物治疗,而雌激素降低、绝经被认为是其病因之一,雄激素常常被视作具有增强肌肉力量的功能。雌激素具有抗氧化和促胶原、血管生成作用,在维护盆底支持结构的力学特性上起到了重要作用。而雄激素可诱导肌纤维肥大、与肌肉力量正相关,但作用可能因性别而存在差异,仍需进一步证实其在女性肌肉力量维持中的作用。目前雌激素和雄激素的替代治疗尚存争议,体外实验大多显示有效,而体内实验则显示矛盾的结果,且激素的补充常常伴随其他靶器官的变化,存在一定风险,仍需设计基础实验和前瞻性实验来作进一步讨论。本文就性类固醇激素对肌肉功能的影响以及可能机制、替代治疗的前景和文献评价进行综述。Pelvic floor dysfunction(PFD),characterized by impaired pelvic floor function,currently lacks globally recognized safe and effective pharmacological treatments.Reduced estrogen levels and menopause are considered one of the etiological factors for PFD,while androgens are often perceived to have a muscle⁃strengthening function.Estrogen exhibits antioxidant properties and promotes collagen and blood vessel formation,playing a crucial role in maintaining the mechanical characteristics of pelvic floor support structures.Whereas androgens,induce muscle fiber hypertrophy and positively correlate with muscle strength,but the effects may differ according to gender,and further confirmation of their role in the maintenance of muscle strength in females is still needed.Controversies persist regarding the replacement therapy of estrogen and androgen,with most in vitro experiments indicating efficacy,while in vivo experiments present conflicting results.Moreover,hormone supplementation often accompanies changes in other target organs,posing certain degree of risks.Further research involving basic experiments and prospective studies is necessary for a more in⁃depth discussion on this matter.This paper provides a comprehensive review of the impact of sex steroid hormones on muscle function,as well as potential mechanisms,prospects of alternative therapies,and literature evaluations.
