桃红四物汤与射频热凝术联用在颈腰椎间盘突出症患者中的治疗效果、经济成本与长期复发率的综合评估研究

摘　　要：目的:探究桃红四物汤与射频热凝术联用在颈腰椎间盘突出症患者中的治疗效果、经济成本与长期复发率的综合效果。方法:选取2021年3月至2023年3月本院收治的152例颈腰椎间盘突出症患者实施前瞻性研究,按照随机数字表法分为研究组76例、对照组76例。对照组采取射频热凝术治疗,研究组采取桃红四物汤与射频热凝术联用治疗,比较两组疗效、疼痛程度、经济学指标、颈腰椎功能(JOA评分)、血清因子水平、椎间盘回缩长度与长期复发率的差异。结果:等级资料秩和检验显示,相较于对照组,研究组疗效与总有效率更好(P<0.05)。相较于对照组,研究组治疗后VAS评分、住院费用更低,住院时间更短(P<0.05)。相较于治疗前,治疗1 w后、治疗后3 m两组JOA评分均升高(P<0.05),且研究组高于对照组(P<0.05)。相较于治疗前,治疗1 w后两组IL-1β、IL-6、TNF-ɑ、CRP水平均降低(P<0.05),且研究组低于对照组(P<0.05)。相较于对照组,研究组治疗后3m椎间盘回缩长度更高,而治疗后1y复发率更低(6.58%vs19.74%)(P<0.05)。结论:桃红四物汤与射频热凝术联合治疗颈腰椎间盘突出症具有较好疗效,可有效缓解疼痛程度,减少治疗费用,改善颈腰椎功能,调节血清因子水平,提升椎间盘回缩长度,降低长期复发率。Objective:Exploring the comprehensive therapeutic effect,economic cost,and long-term recurrence rate of Taohong Siwu Tang combined with radiofrequency thermocoagulation in patients with cervical and lumbar disc herniation.Methods:A prospective study was conducted on 152 patients with cervical and lumbar disc herniation admitted to our hospital from March 2021 to March 2023.They were randomly divided into a study group of 76 cases and a control group of 76 cases using a random number table method.The control group received radiofrequency thermocoagulation treatment,while the study group received a combination of Taohong Siwu Tang and radiofrequency thermocoagulation treatment.The differences in efficacy,pain level,economic indicators,cervical and lumbar spine function(JOA score),serum factor levels,intervertebral disc retraction length,and long-term recurrence rate between the two groups were compared.Results:Rank sum test of rank data showed that compared to the control group,the study group had better efficacy and total effective rate(P<0.05).Compared to the control group,the study group had lower VAS scores and hospitalization costs after treatment,and shorter hospital stay(P<0.05).Compared with before treatment,JOA scores in both groups increased(P<0.05),after 1 week of treatment and 3 months after treatment,and the study group was higher than the control group(P<0.05).Compared to before treatment,after one week of treatment,two groups of IL-1β、IL-6,TNF-ɑ、The CRP levels were all reduced(P<0.05),and the study group was lower than the control group(P<0.05).Compared to the control group,the study group had a higher intervertebral disc retraction length at 3 months after treatment,and a lower recurrence rate at 1 year after treatment(6.58%vs 19.74%)(P<0.05).Conclusion:The combination of Taohong Siwu Tang and radiofrequency thermocoagulation has a good therapeutic effect on cervical and lumbar disc herniation,which can effectively alleviate pain,reduce treatment costs,improve cervical and lumbar spine functio
