摘 要:目的:探析血清白蛋白(Albumin,ALB)、胆碱酯酶(Cholinesterase,CHE)在慢阻肺急性加重期(Acute exacerbation of chronic obstructive pulmonary disease,AECOPD)伴呼吸衰竭通气治疗患者中的表达,并分析二者与患者通气治疗预后的关系。方法:选取2021年1月~2023年12月期间本院确诊并拟行通气治疗的90例AECOPD伴呼吸衰竭患者作为研究对象,通气治疗结束后随访1年,将随访期间发生病死者纳入预后不良组(n=19),其他患者纳入预后良好组(n=71)。检测并比较所有患者入院时(治疗前)、治疗后血清ALB、CHE水平、不同预后情况下AECOPD伴呼入院时血清ALB、CHE水平;并采用限制性立方样条模型、Logistic回归分析入院时血清ALB、CHE与患者通气治疗不良预后的关系。结果:治疗第7d时,AECOPD伴呼吸衰竭患者的血清ALB、CHE水平均较治疗前明显升高(P<0.05)。不良预后组入院时血清ALB、CHE水平均明显低于预后良好组(P<0.05)。经限制性立方样条模型分析,AECOPD伴有呼吸衰竭患者入院时的血清ALB、CHE水平与不良预后风险的关联强度呈非线性剂量反应关系(P<0.05)。经Logistic回归分析,血清ALB、CHE是AECOPD伴有呼吸衰竭患者通气治疗后发生不良预后的保护因素(OR<1,P<0.05)。结论:血清ALB、CHE水平与通气治疗后不良预后风险的关联强度呈非线性剂量反应关系,且血清ALB、CHE是通气治疗预后的保护因素。Objective:To explore the exacerbation of chronic obstructive pulmonary disease in serum Albumin(ALB)and Cholinesterase(CHE)respectively.The expression of AECOPD in patients with respiratory failure combined with ventilation therapy was analyzed,and the relationship between them and the prognosis of patients with ventilation therapy was analyzed.Methods:From January 2021 to December 2023,90 AECOPD patients with respiratory failure who were diagnosed and planned to undergo ventilation treatment in our hospital were selected as the research objects.All patients were followed up for 1 year after ventilation treatment.The patients who died during the follow-up period were included in the poor prognosis group(19 cases),and the other patients were included in the good prognosis group(71 cases).The serum ALB and CHE levels of all patients at admission(before treatment)and after treatment,and the serum ALB and CHE levels of AECOPD patients with different prognosis at admission were detected and compared.The restrictive cubic spline model and Logistic regression were used to analyze the relationship between serum ALB and CHE at admission and the poor prognosis of patients with ventilation treatment.Results:On the 7 th day of treatment,the levels of serum ALB and CHE in AECOPD patients with respiratory failure were significantly higher than those before treatment(P<0.05).The levels of serum ALB and CHE in the poor prognosis group were significantly lower than those in the good prognosis group(P<0.05).According to the analysis of restricted cubic spline model,the correlation between serum ALB and CHE levels at admission and the risk of poor prognosis in AECOPD patients with respiratory failure was nonlinear dose-response relationship(P<0.05).Logistic regression analysis showed that serum ALB and CHE were protective factors for poor prognosis in AECOPD patients with respiratory failure after ventilation treatment(OR<1,P<0.05).Conclusions:There was a non-linear dose-response relationship between serum ALB and CHE levels and th
