摘　　要：目的 探讨血浆甘油三酯-葡萄糖(TyG)指数与老年人急性心肌梗死(AMI)的关系。方法 选取2007年10月~2011年7月解放军总医院老年心血管内科收治的987例老年冠心病患者的临床资料。以发作性胸痛为首发症状,根据是否发生急性心肌梗死分为AMI组(n=68)和非AMI组(n=919)。比较两组患者的临床特征,采用二元logistic回归和受试者工作特征(ROC)曲线分析TyG指数与老年冠心病患者AMI发生的关系以及对AMI发生风险的预测价值。结果 多因素logistic回归模型分析结果显示,校正多种混杂因素后,体质量指数(BMI)、吸烟、TyG指数均是AMI发生的独立危险因素(P<0.05)。TyG指数用于预测冠心病患者发生AMI风险的ROC曲线下面积(AUC)为0.614(95%CI=0.547~0.681,P=0.002),预测AMI的最佳截断值为8.699,灵敏度为67.6%,特异度为57%,约登指数为0.246。结论 TyG指数与老年人发生AMI的风险独立关联,可作为预测老年人群发生AMI的风险指标。Objective To analyze the correlation between triglyceride glucose(TyG)index and acute myocardial infarction(AMI)in elderly patients.Methods A total of 987 elderly patients with coronary heart disease who were admitted to the cardiology department of our hospital from October 2007 to July 2011 were enrolled in the study.According to whether they had AMI or not,the patients were divided into the non-AMI group(n=919)or the AMI group(n=68).Receiver operating characteristic(ROC)curve and Logistic regression were used to analyze the predictive value of TyG index for AMI.Results After adjusting for all potential confounders,multivariate Logistic regression analysis showed that the TyG index(OR=0.039,P<0.05)was an independent risk factor for AMI.The area under the curve(AUC)of TyG index for predicting the risk of AMI in patients with coronary heart disease was 0.614(95%CI=0.547~0.681,P=0.002)and the optimal cut-off value was 8.6992(the sensitivity was 67.6%,the specificity was 57%,and the Youden index was 0.246).Conclusion TyG index is an independent risk factor for AMI.TyG index are expected to provide a predictive value for elderly AMI patients.
