彩超下中晚孕期胎儿三尖瓣返流相关参数对围生儿结局的预测效能探究

摘　　要：目的探究彩超下中晚孕期胎儿三尖瓣返流(TR)相关参数对围生儿结局的预测效能。方法回顾性分析2020年2月~2023年2月我院收治的经彩超确诊为TR的中晚孕期产妇及胎儿94例,依据围生儿结局不同分为不良结局组13例,正常结局组81例。比较两组彩超相关参数的差异,并判断其预测围生儿结局的价值。结果不良结局组:胎儿大脑中动脉(MCA)阻力指数(RI)、搏动指数(PI)、收缩期与舒张期流速比(S/D)均低于正常结局组,脐动脉(UA)RI、PI、S/D均高于正常结局组(P<0.05);TR胎儿MCA、UA的RI、PI、S/D的AUC均>0.7,对围生儿不良结局的预测均具有一定的价值(P<0.05)。结论应用彩超检测中晚孕期TR胎儿MCA、UA的RI、PI、S/D参数对于预测围生儿不良结局具有一定的价值,临床上可据此进行不良预后的防治。Objective To investigate the predictive efficacy of parameters related to fetal tricuspid regurgitation(TR)in perinatal outcomes during middle and late pregnancy under color ultrasound.Methods From February 2020 to February 2023,94 pregnant women and fetuses diagnosed with TR by color ultrasound in our hospital were retrospectively analyzed,and were divided into 13 cases of adverse outcome group and 81 cases of normal outcome group according to different perinatal outcomes.The difference of relevant parameters of color ultrasound between two groups was compared,and its value in predicting perinatal outcomes was evaluated.Results In the adverse outcome group,the resistance index(RI),pulsation index(PI),systolic to diastolic flow velocity ratio(S/D)of the middle cerebral artery(MCA)were lower than those in the normal outcome group,and the umbilical artery(UA)RI,PI,and S/D were higher than those in the normal outcome group(P<0.05).The RI,PI and AUC of MCA and UA of TR fetus were all>0.7,which had certain value in predicting adverse outcomes of perinatal infants(P<0.05).Conclusion The application of color ultrasound to detect the RI,PI and S/D parameters of MCA and UA of TR fetus in the middle and late pregnancy has certain value in predicting the adverse outcome of perinatal infants,and can be used to prevent and treat the adverse prognosis in clinic.
