摘  要:目的探究世居藏族膝骨关节炎患者血清和关节液中的34项细胞因子水平其临床意义。方法随机选取2023年5月~2023年8月进行全膝置换术的KOA患者30例。采用Luminex多因子检测,患者血清(血清组)及关节液(关节组)细胞因子的表达水平;采用视觉模拟评分法(VAS)和西大略湖麦克马斯特大学骨关节炎指数(WOMAC)评估关节疼痛程度和功能障碍程度。并分析细胞因子与VAS和WOMAC评分的相关性。结果两组细胞因子:趋化因子类SDF-1α、IP-10、MIP-1β、MCP-1的浓度水平(P>0.05);两组SDF-1α、IP-10、Eotaxin、MIP-1β呈正相关(P<0.05);关节液SDF-1α和MCP-1水平与患者WOMAC评分呈正相关(P<0.05),IP-10与患者C-反应蛋白、ESR呈正相关(P<0.05);关节液中的Eotaxin水平与BMI呈负相关。结论世居藏族KOA患者晚期关节液和外周血中产生了趋化因子,这些趋化因子可能会对早期KOA的诊断与治疗提供参考依据。Objective To investigate the clinical significance of 34 cytokines levels in serum and joint fluid of Tibetan patients with knee osteoarthritis.Methods Thirty patients with KOA who underwent total knee replacement from May 2023 to August 2023 were randomly selected.Luminex multifactor assay was used to detect the cytokine expression levels in serum(serum group)and joint fluid(joint group)of patients.The visual analogue scale(VAS)and the McMaster University Western Ontario Osteoarthritis Index(WOMAC)were used to assess the degree of joint pain and dysfunction.The correlation between cytokines and VAS and WOMAC scores was analyzed.Results The concentrations of cytokines SDF-1α,IP-10,MIP-1βand MCP-1 in two groups(P>0.05);SDF-1α,IP-10,Eotaxin and MIP-1βwere positively correlated between the two groups(P<0.05).Alpha and MCP joint fluid SDF-1-1 level and patient WOMAC scores were positively correlated(P<0.05),IP-10 was positively correlated with patients'C-reactive protein,the ESR(P<0.05);Eotaxin levels in joint fluid were negatively correlated with BMI.Conclusion Chemokines are produced in joint fluid and peripheral blood of Tibetan KOA patients in the late stage,which may provide reference for the early diagnosis and treatment of KOA.
