镇静镇痛不足导致人机对抗案例分析

摘　　要：机械通气中的人机对抗,指的是患者自主呼吸与机械通气之间的不协调现象。这种人机对抗会导致患者呼吸肌过度做功,气道压力上升,通气量下降,进而影响血流动力学的稳定性。因此,在机械通气过程中,镇静镇痛措施显得尤为关键。镇静镇痛与患者个人体质、脾气、性格等自身因素密切相关,因此镇静镇痛药物的选择及用量对于机械通气患者尤为重要。本研究通过对五个案例进行探究,分析机械通气镇静镇痛的使用策略及药物选择,以期达到最佳的治疗效果,同时最大程度地减少患者的不适感和可能产生的副作用。Human machine confrontation in mechanical ventilation refers to the phenomenon of disharmony between patients'spontaneous breathing and mechanical ventilation.This human-machine confrontation can lead to excessive work done by the patient's respiratory mus⁃cles,increased airway pressure,decreased ventilation,and thus affect the stability of hemodynamics.Therefore,sedative and analgesic mea⁃sures are particularly crucial during mechanical ventilation.Sedative and analgesic drugs are closely related to the patient's personal consti⁃tution,temperament,personality,and other personal factors.Therefore,the selection and dosage of sedative and analgesic drugs are particu⁃larly important for mechanically ventilated patients.This study explores the use strategies and drug selection of mechanical ventilation seda⁃tion and analgesia through five cases,in order to achieve the best therapeutic effect while minimizing patient discomfort and potential side effects.
