摘 要:目的分析血清微小RNA(miR)-93-5p、第10号染色体上缺失的磷酸酶和紧张素同源物(PTEN)表达水平在评价川崎病(KD)患儿冠状动脉损伤(CAL)中的应用价值。方法回顾性分析2021年10月至2023年12月郑州大学附属儿童医院心血管内科收治的164例KD患儿的临床诊治资料,依据冠状动脉超声检查结果分为KD+CAL组56例和KD组108例。比较两组患儿的一般资料、血清miR-93-5p和PTEN水平,采用Pearson相关分析法分析血清miR-93-5p与PTEN水平的相关性,采用受试者工作特征(ROC)曲线分析血清miR-93-5p、PTEN水平对KD患儿CAL的诊断价值,采用多因素Logistic回归法分析KD患儿发生CAL的影响因素。结果KD+CAL组患儿血小板计数、发热持续时间、静脉注射用免疫球蛋白使用时间、降钙素原(PCT)、C反应蛋白(CRP)及血清miR-93-5p水平分别为(430.33±86.42)×10^(9)/L、(9.84±1.96)d、(9.67±1.92)d、(0.68±0.13)μg/L、(82.46±16.24)mg/L、1.51±0.36,明显高于KD组的(351.61±70.18)×10^(9)/L、(8.11±1.07)d、(6.53±1.18)d、(0.32±0.06)μg/L、(65.67±13.25)mg/L、1.02±0.28,血清PTEN水平为(2.32±0.41)ng/mL,明显低于KD组的(3.03±0.61)ng/mL,差异均有统计学意义(P<0.05);经Pearson相关分析法分析结果显示,KD+CAL组患儿血清miR-93-5p与PTEN呈负相关(r=-0.522,P<0.05);经ROC曲线分析结果显示,血清miR-93-5p、PTEN水平联合诊断KD患儿CAL的曲线下面积(AUC)为0.917(95%CI:0.863~0.954),明显高于miR-93-5p单独诊断的AUC 0.837(95%CI:0.771~0.890)、PTEN单独诊断的AUC 0.847(95%CI:0.783~0.899),差异均有统计学意义(P<0.05);经Logistic回归法分析结果显示,血小板计数、发热持续时间、静脉注射用免疫球蛋白使用时间及血清miR-93-5p、PTEN水平均是KD患儿发生CAL的影响因素(P<0.05)。结论KD合并CAL患儿血清miR-93-5p水平上调,血清PTEN水平下调,检测其水平变化可用于疾病的早期诊断。Objective To analyze the application value of serum miRNA(miR)935p,phosphatase and tensin homology deleted on chromosome 10(PTEN)expression levels in evaluating coronary artery lesion(CAL)in children with Kawasaki disease(KD).Methods The clinical diagnosis and treatment data of 164 children with KD admitted to the Department of Cardiovascular Medicine,Children's Hospital Affiliated to Zhengzhou University from October 2021 to December 2023 were retrospectively analyzed.Based on the results of coronary artery ultrasound examination,the patients were assigned into KD+CAL group(56 cases)and KD group(108 cases).The general data,serum miR935p,and PTEN levels of the two groups were compared.Pearson correlation analysis was applied to analyze the correlation between serum miR935p and PTEN levels.Receiver operating characteristic(ROC)curve was applied to analyze the diagnostic value of serum miR935p and PTEN levels for CAL in children with KD.Multivariate logistic regression was applied to analyze the influencing factors of CAL in children with KD.Results The platelet count,duration of fever,duration of intravenous immunoglobulin administration,procalcitonin(PCT),Creactive protein(CRP),and serum miR935p levels in the KD+CAL group were(430.33±86.42)×10^(9)/L,(9.84±1.96)d,(9.67±1.92)d,(0.68±0.13)μg/L,(82.46±16.24)mg/L,and 1.51±0.36,respectively,which were significantly higher than(351.61±70.18)×10^(9)/L,(8.11±1.07)d,(6.53±1.18)d,(0.32±0.06)μg/L,(65.67±13.25)mg/L,and 1.02±0.28 in the KD group;the serum PTEN level in the KD+CAL group was(2.32±0.41)ng/mL,which was significantly lower than(3.03±0.61)ng/mL in the KD group;the differences were statistically significant(P<0.05).Pearson correlation analysis showed that serum miR935p and PTEN levels in KD+CAL group were negatively correlated(r=0.522,P<0.05).ROC curve analysis showed that the area under the curve(AUC)for the combined diagnosis of CAL in children with KD by serum miR935p and PTEN levels was 0.917(95%CI:0.8630.954),which were significantly higher tha
