摘 要:目的分析增强扫描肺动脉CT血管造影技术(CTPA)对儿童支原体肺炎合并肺动脉栓塞的诊断价值。方法回顾性收集河南省儿童医院郑州儿童医院2019年1月至2021年12月临床诊断为支原体肺炎合并肺动脉栓塞且行胸部CT双期增强检查的78例患儿(对照组)及2021年8月至2022年10月临床诊断为支原体合并肺动脉栓塞且行CTPA检查的62例患儿(研究组)的临床资料,比较两组患儿的客观图像质量、主观图像质量、辐射剂量、肺动脉栓塞检出率和动脉狭窄检出情况。结果研究组患儿的图像CT值和噪声分别为(418.69±74.23)Hu、(27.86±3.12)dB,明显高于对照组的(391.25±70.42)Hu、(24.95±3.98)dB,对比噪声比为17.69±3.14,明显低于对照组的19.92±3.24,差异均有统计学意义(P<0.05);研究组患儿的主观图像质量评分明显优于对照组,差异有统计学意义(P<0.05);研究组患儿的放射剂量长度乘积(DLP)、容积CT剂量指数(CTDIvol)、有效辐射剂量(ED)分别为(91.53±15.84)mGy/cm、(2.80±0.49)mGy、(1.37±0.25)mSv,明显低于对照组的(148.57±52.31)mGy/cm、(4.41±1.37)mGy、(2.20±0.29)mSv,差异均有统计学意义(P<0.05);研究组患儿的整体肺栓塞检出率为96.77%,明显高于对照组的80.77%,差异有统计学意义(P<0.05);研究组患儿的中度动脉狭窄、重度动脉狭窄、闭塞及总动脉狭窄的检出率明显高于对照组,差异均有统计学意义(P<0.05)。结论增强扫描CTPA在儿童支原体肺炎的诊断中能够获得较佳的图像质量,且能够降低辐射剂量,提高肺动脉栓塞和动脉狭窄检出率。Objective To analyze the diagnostic value of contrastenhanced computed tomography pulmonary angiogram(CTPA)in children with Mycoplasma pneumonia complicated with pulmonary embolism.Methods The clinical data of 78 children(control group)who were clinically diagnosed as Mycoplasma pneumonia complicated with pulmonary embolism and underwent dualphase enhanced chest CT examination in Zhengzhou Children's Hospital,Henan Children's Hospital from January 2019 to December 2021 and 62 children(study group)who were clinically diagnosed as Mycoplasma pneumonia complicated with pulmonary embolism and underwent CTPA examination from August 2021 to October 2022 were retrospectively collected.The objective image quality,subjective image quality,radiation dose,pulmonary embolism detection rate,and arterial stenosis detection were compared between the two groups.Results The CT values and noise of the children in the study group were(418.69±74.23)Hu and(27.86±3.12)dB,respectively,which were significantly higher than(391.25±70.42)Hu and(24.95±3.98)dB in the control group(P<0.05),and the contrasttonoise ratio was 17.69±3.14,significantly lower than 19.92±3.24 in the control group(P<0.05).The subjective image quality score in the study group was significantly better than that of the control group(P<0.05).The dose length product(DLP),CT dose index(CTDIvol),and effective dose(ED)in the study group were(91.53±15.84)mGy/cm,(2.80±0.49)mGy,and(1.37±0.25)mSv,which were significantly lower than(148.57±52.31)mGy/cm,(4.41±1.37)mGy,(2.20±0.29)mSv in the control group(P<0.05).The overall detection rate of pulmonary embolism in the study group was 96.77%,which was significantly higher than 80.77%in the control group(P<0.05).The detection rates of moderate arterial stenosis,severe arterial stenosis,and occlusion in the study group were significantly higher than those in the control group(P<0.05).Conclusion Contrastenhanced CTPA can obtain better image quality in the diagnosis of Mycoplasma pneumonia in children,reduce radiation dose,an
