痔疮手术治疗后凉血地黄汤联合针灸对疗效的影响

摘　　要：目的分析痔疮手术治疗后使用凉血地黄汤联合针灸对疗效的影响。方法选取2021年10月—2023年10月在胶州市中医医院普外科接受手术治疗的138例痔疮患者为研究对象,根据不同的治疗方法分为对照组(n=69,常规方案治疗)、研究组(n=69,凉血地黄汤联合针灸治疗)。比较两组疼痛评分、创面愈合指标(转化生长因子-β、表皮生长因子、Ⅰ型胶原蛋白)、术后康复时间(肛门水肿消失时间、渗出消失时间、创面愈合时间)、中医证候积分(面色淡白、体倦乏力、烦躁不安)。结果 治疗1、3、5 d后,研究组视觉模拟量表评分均低于对照组,术后康复时间均短于对照组,面色淡白、体倦乏力、烦躁不安积分低于对照组,差异有统计学意义(P均<0.05)。研究组转化生长因子-β为(0.74±0.12)μg/L、表皮生长因子为(0.79±0.26)μg/L、Ⅰ型胶原蛋白为(27.51±5.11)μg/L,均高于对照组的(0.55±0.09)μg/L、(0.49±0.13)μg/L、(19.14±4.96)μg/L,差异有统计学意义(t=10.522、8.573、9.763,P均<0.05)。结论 痔疮术后以凉血地黄汤配合针灸对患者进行治疗,可以镇痛、缓解临床症状,促进康复。Objective To analyze the effect of Liangxue Dihuang Decoction combined with acupuncture on the curative effect after hemorrhoid surgery.Methods A total of 138 patients with hemorrhoids who underwent surgical treatment in the Department of General Surgery of Jiaozhou Hospital of Traditional Chinese Medicine from October 2021 to October 2023 were selected as the research objects.According to different treatment methods,they were divided into control group(n=69,conventional treatment) and study group(n=69,Liangxue Dihuang Decoction combined with acupuncture treatment).The pain scores,wound healing indexes(transforming growth factor-β,epidermal growth factor,collagen type I),postoperative rehabilitation time(anal edema disappearance time,exudation disappearance time,wound healing time),and Traditional Chinese Medicine syndrome scores(pale face,tiredness and fatigue,irritability) were compared between the two groups.Results After 1,3and 5 d of treatment,the Visual Analog Scale scores of the study group were lower than those of the control group,the postoperative recovery time were shorter than that of the control group,and the scores of pale face,tiredness and fatigue,and irritability were lower than those of the control group,and the differences were statistically significant(all P<0.05).In the study group,transforming growth factor-β was(0.74±0.12) μg/L,epidermal growth factor was(0.79±0.26) μg/L,and collagen type I was(27.51±5.11) μg/L,which were higher than those of the control group,which were(0.55±0.09) μg/L,(0.49±0.13) μg/L,and(19.14±4.96) μg/L,and the differences were statistically significant(t=10.522,8.573,9.763,all P<0.05).Conclusion Treating patients with Liangxue Dihuang Decoction together with acupuncture after hemorrhoids surgery can analgesic,relieve the clinical symptos and promote recovery.
