摘 要:目的 探讨四维彩超检查在产前胎儿畸形检查中的应用价值。方法 选取2021年12月—2023年12月在新疆石河子妇幼保健院进行产前胎儿畸形检查的80例孕妇为研究对象,所有孕妇在孕中期(20~24周)进行四维彩超检查,并将综合检查作为金标准,分析四维彩超检查对胎儿畸形的诊断效能。结果 综合检查结果显示,80例孕妇中有13例胎儿畸形,占比为16.25%(13/80)。四维彩超检查检出真阳性11例,真阴性65例。四维彩超检查与综合检查结果诊断胎儿畸形的kappa值为0.857,一致性良好。四维彩超检查产前畸形的准确度、灵敏度、特异度、阳性预测值和阴性预测值分别是95.00%(76/80)、84.62%(11/13)、97.01%(65/67),84.62%(11/13)、97.01%(65/67)。四维彩超检查诊断四肢畸形、颜面畸形、心脏畸形、神经系统畸形、肺部畸形的符合率分别为100.00%(3/3)、100.00%(4/4)、50.00%(1/2)、100.00%(2/2)和50.00%(1/2)。结论 四维彩超检查对胎儿畸形有较高的诊断效能,对产前筛查胎儿畸形有一定的临床价值。Objective To explore the application value of four dimensional color ultrasonography in prenatal fetal malformation examination.Methods A total of eighty pregnant women who underwent prenatal fetal malformation examination in Xinjiang Shihezi Maternal and Child Health Hospital from December 2021 to December 2023 were selected as the study objects.All pregnant women underwent four dimensional color ultrasound examination in the second trimester(20-24 weeks),and comprehensive examinations were used as the gold standard.The diagnostic efficiency of four dimensional color ultrasonography in the diagnosis of fetal malformation was analyzed.Results The comprehensive examinations results showed that among the 80 pregnant women,13 cases of fetal malformation,accounting for 16.25%(13/80).The true positive cases and true negative cases were 11 and 65 respectively.The kappa value for the diagnosis of fetal malformation by four dimensional color ultrasonography and the comprehensive examinations results was 0.857,and the consistency was good.The accuracy,sensitivity,specificity,positive predictive value and negative predictive value of prenatal deformities of four dimensional color ultrasound were 95.00%(76/80),84.62%(11/13),97.01%(65/67),84.62%(11/13),97.01%(65/67).The coincidence rates of four dimensional color ultrasonography were 100.00%(3/3),100.00%(4/4),50.00%(1/2),100.00%(2/2) and 50.00%(1/2) for the diagnosis of limb malformations,facial malformations,heart malformations,nervous system malformations and lung malformations.Conclusion The four dimensional color ultrasonography has high diagnostic efficiency in the diagnosis of fetal malformations,and has certain clinical value in prenatal screening of fetal malformations.
