颈动脉超声联合经颅多普勒诊断短暂性脑缺血发作的临床价值

摘　　要：目的 分析短暂性脑缺血发作应用颈动脉超声联合经颅多普勒诊断的应用效果。方法 选择2022年5—12月赤峰松山医院收治的100例疑似短暂性脑缺血发作患者为研究对象,所有患者均采取颈动脉超声联合经颅多普勒诊断,以脑血管造影术为金标准。分析颈动脉超声联合经颅多普勒诊断的灵敏度、特异度及准确度。结果 脑血管造影术检出阳性80例,阴性20例。经颅多普勒超声联合颈动脉超声诊断的灵敏度为98.75%(79/80)、准确度为98.00%(98/100)、特异度为95.00%(19/20),与脑血管造影术结果一致性高(kappa=0.938)。结论 颈动脉超声联合经颅多普勒诊断可以清楚显示患者颈动脉和基底动脉的血液动力学状态,方便医师判断颈动脉狭窄和内膜增厚的病变。Objective To analyze the application effect of carotid ultrasound combined with transcranial Doppler in the diagnosis of transient ischemic attack.Methods A total of 100 patients with suspected transient ischemic attack admitted to Chifeng Songshan Hospital from May to December 2022 were selected as the research objects.All patients were diagnosed by carotid ultrasound combined with transcranial Doppler,and cerebral angiography was used as the gold standard.The sensitivity,specificity and accuracy of carotid ultrasound combined with transcranial Doppler were analyzed.Results Cerebral angiography detected positive in 80 cases,negative in 20 cases.The sensitivity,accuracy and specificity of carotid ultrasound combined with transcranial Doppler diagnosis were 98.75%(79/80),98.00%(98/100) and 95.00%(19/20),respectively,which were highly consistent with the results of cerebral angiography(kappa=0.938).Conclusion The combination of transcranial Doppler and carotid ultrasound can clearly show the hemodynamic status of carotid artery and basilar artery,which is convenient for doctors to judge the lesions of carotid stenosis and intimal thickening.
