经尿道前列腺钬激光剜除术与尿道2μm激光前列腺切除术治疗良性前列腺增生的临床效果

摘　　要：目的分析良性前列腺增生实施经尿道前列腺钬激光剜除术与尿道2μm激光前列腺切除术治疗的效果。方法单纯随机选取2021年3月—2023年6月北京航天总医院收治的140例良性前列腺增生患者为研究对象,根据术式的不同将其分为参照组与治疗组,各70例。参照组采用经尿道前列腺钬激光剜除术治疗,治疗组采用尿道2μm激光前列腺切除术治疗。对比两组患者的手术相关指标和功能指标。结果治疗组的手术时间长于参照组,术中出血量少于参照组,膀胱冲洗时间、置管时间以及住院时间短于参照组,差异有统计学意义(P均<0.05)。术前,两组国际勃起功能指数、国际前列腺症状评分对比,差异无统计学意义(P均>0.05);随访3个月后,两组国际勃起功能指数、国际前列腺症状评分均降低,且治疗组评分(10.12±1.08)分、(4.90±1.25)分均低于参照组的(11.02±0.67)分、(6.77±1.34)分,差异有统计学意义(t=5.925、8.538,P均<0.05)。结论相较于经尿道前列腺钬激光剜除术,虽然尿道2μm激光前列腺切除术治疗良性前列腺增生的手术过程用时稍长,但术中出血量较低,并且患者术后的恢复时间更为迅速,可更好地缓解症状,改善性功能。Objective Analysis of the effectiveness of transurethral holmium laser enucleation of the prostate versus urethral 2μm laser prostatectomy for benign prostatic hyperplasia.Methods A total of 140 patients with benign pros-tatic hyperplasia admitted to Beijing Aerospace General Hospital from March 2021 to June 2023 were simple ran-domly selected as the research objects.According to the different surgical methods,they were divided into reference group and treatment group,with 70 cases in each group.The reference group was treated with transurethral holmium laser enucleation of the prostate,and the treatment group was treated with transurethral 2μm laser prostatectomy.The operation related indexes and functional indexes of the two groups were compared.Results The operation time of the treatment group was longer than that of the reference group,the intraoperative blood loss was less than that of the ref-erence group,the bladder irrigation time,catheterization time and hospitalization time were shorter than those of the reference group,and the differences were statistically significant(all P<0.05).Before operation,there was no signifi-cant difference in international index of erectile function and international prostate symptom score between the two groups(both P>0.05).After 3 months of follow-up,the international index of erectile function and international pros-tate symptom score of the two groups decreased,and the scores of the treatment group(10.12±1.08)points and(4.90±1.25)points,which were lower than(11.02±0.67)points and(6.77±1.34)points of the reference group,the differences were statistically significant(t=5.925,8.538,both P<0.05).Conclusion Compared with transurethral holmium laser enucleation of the prostate,although the operation time of urethral 2μm laser prostatectomy is longer,the intraopera-tive blood loss is lower,and the patient's postoperative recovery time is faster.It can better relieve symptoms and im-prove sexual function.
