对比分析小针刀微创手术与传统手术治疗拇指屈指肌腱狭窄性腱鞘炎的效果

摘　　要：目的分析小针刀微创手术与传统手术治疗拇指屈指肌腱狭窄性腱鞘炎的疗效。方法随机选择2020年11月—2022年11月晋江市医院收治的80例拇指屈指肌腱狭窄性腱鞘炎患者为研究对象。根据手术方法不同分组,每组40例。对照组行传统手术治疗,观察组行小针刀微创手术治疗,比较两组患者的手术时间、手术切口长度、术中出血量、创口愈合时间、手术效果、掌指关节处(弹响、绞索、硬结)情况、术后并发症、生活质量评分。结果两组的治疗总有效率比较,差异无统计学意义(P>0.05)。观察组的出血量、切口长度及创口愈合时间均优于对照组,差异有统计学意义(P均<0.05);两组手术时间比较,差异无统计学意义(P>0.05)。观察组掌指关节情况优于对照组,差异有统计学意义(P<0.05)。观察组的术后生活质量评分高于对照组,差异有统计学意义(P<0.05)。观察组并发症总发生率为5.00%(2/40),低于对照组的30.00%(12/40),差异有统计学意义(χ^(2)=8.658,P<0.05)。结论小针刀微创手术治疗拇指屈指肌腱狭窄性腱鞘炎的安全性高,手术损伤小、术后恢复快,术后并发症少,整体手术情况良好。Objective To analyze the curative effect of small needle knife minimally invasive surgery and traditional surgery in the treatment of thumb flexor tendon stenosis tenosynovitis.Methods A total of 80 patients with thumb flexor tendon stenosis tenosynovitis admitted to Jinjiang Hospital from November 2020 to November 2022 were ran-domly selected as the research objects.Grouped according to different surgical methods,40 cases in each group.The control group was treated with traditional surgery,and the observation group was treated with small needle knife mini-mally invasive surgery.The operation time,length of surgical incision,bleeding volume,wound healing time,surgical effect,metacarpophalangeal joint(snapping,noose,induration),postoperative complications and quality of life scores were compared between the two groups.Results There was no significant difference in the total effective rate of treat-ment between the two groups(P>0.05).The bleeding volume,incision length and wound healing time of the observa-tion group were better than those of the control group,and the differences were statistically significant(all P<0.05).There was no significant difference in operation time between the two groups(P>0.05).The metacarpophalangeal joint of the observation group was better than that of the control group,and the difference was statistically significant(P<0.05).The postoperative quality of life score of the observation group was higher than that of the control group,and the difference was statistically significant(P<0.05).The total incidence of complications in the observation group was 5.00%(2/40),which lower than that in the control group 30.00%(12/40),and the difference was statistically significant(χ^(2)=8.658,P<0.05).Conclusion Small needle knife minimally invasive for thumb flexor tendon stenosis tenosynovitis has high safety,small surgical injury,rapid postoperative recovery,few postoperative complications,and the over-all operation is in good condition.
