H型高血压合并急性脑梗死与颈动脉内膜中层厚度的相关性分析

摘　　要：目的分析H型高血压合并急性脑梗死(acute cerebral infarct,ACI)与颈动脉内膜中层厚度(carotid intima-media thickness,CIMT)的相关性。方法选择南阳市中心医院2021年1月至2023年12月收治的90例ACI患者,据同型半胱氨酸(homocysteine,Hcy)水平及有无高血压将其分为A组(H型高血压合并ACI组)、B组(高血压合并ACI组)、C组(单纯ACI组),每组30例,另选同期来本院体检的健康人群30例为D组,检测4组患者Hcy、可溶性细胞间黏附分子-1(soluble intercellular adhesion molecule-1,sICAM-1)、白细胞介素-6(interleukin-6,IL-6)、白细胞介素-8(interleukin-8,IL-8)、单核细胞趋化因子-1(monocyte chemotactic protein 1,MCP-1)、甲壳质酶蛋白40(chitinase protein 40,YKL-40)水平,采用彩色多普勒超声诊断仪测定CIMT,分析Hcy水平与CIMT的相关性。结果D组、C组、B组sICAM-1、IL-6、IL-8、MCP-1、YKL-40水平高于A组,组间对比差异有统计学意义(P<0.05);D组、C组sICAM-1、IL-6、IL-8、MCP-1、YKL-40水平高于B组,组间对比差异有统计学意义(P<0.05);D组sICAM-1、IL-6、IL-8、MCP-1、YKL-40水平高于C组,组间对比差异有统计学意义(P<0.05);D组、C组、B组Hcy水平高于A组,CIMT大于A组,组间对比差异有统计学意义(P<0.05);D组、C组Hcy水平高于B组,CIMT大于B组,组间对比差异有统计学意义(P<0.05);D组Hcy水平高于C组,CIMT大于C组,组间对比差异有统计学意义(P<0.05);Pearson相关性分析显示,H型高血压合并ACI组Hcy水平与CIMT呈正相关(P<0.05)。结论Hcy水平升高是导致CIMT增厚的重要因素,Hcy水平可作为评估H型高血压合并ACI患者预后的重要指标,降低Hcy水平可改善患者预后,延缓颈动脉粥样硬化进展,降低ACI发生风险。Objective To analyze the correlation between H-type hypertension complicated by acute cerebral infarct(ACI)and carotid intima-media thickness(CIMT).Methods Totally 90 patients with ACI in Nanyang Central Hospital from January 2021 to December 2023 were selected and divided into group A(H-type hypertension combined with ACI group),group B(hypertension combined with ACI group)and group C(ACI group alone)according to their homocysteine(Hcy)level and whether they had hypertension.Another 30 healthy people who came to Nanyang Central Hospital for physical examination at the same period were selected as group D,and Hcy,soluble intercellular adhesion molecule-1(sICAM-1)and interleukin-6(IL-6)of the four groups were detected.IL-6,interleukin-8(IL-8),monocyte chemotactic protein 1(MCP-1),chitinase protein 40(YKL-40),monocyte chemotactic protein 1(MCP-1)level was measured by color Doppler ultrasonography,and the correlation between Hcy level and CIMT was analyzed.Results The levels of sICAM-1,IL-6,IL-8,MCP-1 and YKL-40 in groups D,C and B were higher than those in group A,the difference between groups was statistically significant(P<0.05).The levels of sICAM-1,IL-6,IL-8,MCP-1 and YKL-40 in groups D and C were higher than those in group B,the difference between groups was statistically significant(P<0.05).The levels of sICAM-1,IL-6,IL-8,MCP-1 and YKL-40 in group D were higher than those in group C,the difference between groups was statistically significant(P<0.05).The Hcy level in groups D,C and B was higher than that in group A,and the CIMT was higher than that in group A,the difference between groups was statistically significant(P<0.05).The Hcy level in groups D and C was higher than that in group B,and the CIMT was higher than that in group B,the difference between groups was statistically significant(P<0.05).The Hcy level of group D was higher than that of group C,and the CIMT was higher than that of group C,the difference between groups was statistically significant(P<0.05).Pearson correlation analysis showed that the
