可延长外固定支架在胫骨平台骨折围手术期应用的临床疗效观察

摘　　要：目的观察SchatzkerⅤ、Ⅵ型胫骨平台骨折围手术期应用可延长外固定支架临时固定的临床疗效。方法回顾性分析自2015-12—2018-12诊治的46例SchatzkerⅤ、Ⅵ型胫骨平台骨折,根据围手术期处理方式不同分为可延长外固定架组(23例)和骨牵引组(23例)。比较可延长外固定架组与骨牵引组术前等待时间、住院时间、手术时间、血栓形成发生率,以及术后12 h、48 h、72 h疼痛VAS评分。结果46例均获得随访,随访时间12~30个月,平均22.2个月。可延长外固定架组术前等待时间、住院时间、手术时间均少于骨牵引组,差异有统计学意义(P<0.05)。可延长外固定架组血栓形成发生率低于骨牵引组,差异有统计学意义(P<0.05)。可延长外固定支架组术后12 h、48 h、72 h疼痛VAS评分均低于骨牵引组,差异有统计学意义(P<0.05)。结论SchatzkerⅤ、Ⅵ型胫骨平台骨折围手术期应用可延长外固定支架可减轻患者疼痛,减少术前等待时间、手术时间和住院时间,便于围手术期间护理,有助于后期骨折复位,符合加速康复外科理念。Objective To observe the clinical effect of an extendable external fixator in temporary fixation of Schatzker typeⅤandⅥtibial plateau fractures in the perioperative period.Methods A retrospective analysis was performed on 46 cases of Schatzker typeⅤorⅥtibial plateau fractures diagnosed and treated from December 2015 to December 2018,and they were divided into extendable external fixator group(23 cases)and bone traction group(23 cases)according to perioperative management.Preoperative waiting time,hospital stay,operation time,thrombosis rate,and pain VAS score at 12 h,48 h and 72 h after surgery were compared between the extendable external fixator group and the bone traction group.Results All 46 cases were followed up for 12 to 30 months(mean 22.2 months).The preoperative waiting time,hospital stay and operation time of the extendable external fixator group were less than those of the bone traction group,and the differences were statistically significant(P<0.05).The thrombosis rate in the extendable external fixator group was lower than that in the bone traction group,and the difference was statistically significant(P<0.05).Pain VAS scores at 12 h,48 h and 72 h in the extendable external fixator group were lower than those in the bone traction group,and the difference was statistically significant(P<0.05).Conclusion Perioperative application of the extendable external fixator for Schatzker typeⅤandⅥtibial plateau fractures can reduce the pain of patients,the preoperative waiting time,operation time and hospital stay,and facilitate perioperative nursing and the fracture reduction in the later stage,which conforms to the concept of accelerated rehabilitation surgery.
