摘  要:目的探讨颞骨高分辨率CT测量正常听力者内耳各解剖结构的测量方法和临床应用价值。方法回顾性分析2019年至2021年就诊于郑州大学第一附属医院耳科的100例听力正常患者(200耳)的高分辨率CT图像资料,于轴位图像上测量上半规管骨岛宽度、后半规管骨岛宽度、外半规管骨岛宽度、前庭长度及宽度、耳蜗底转长度及宽度、上底转长度、中转长度、耳蜗高度,冠状位图像上测量耳蜗高度及宽度。比较各测量指标在性别、左右耳间的差异。结果所有受检者高分辨率CT均未见内耳及中耳结构异常,前庭导水管不可见或成线状,中点宽度均<1.5 mm。内耳各结构测量正常值范围:上半规管骨岛宽度为(5.71±0.36)mm,后半规管骨岛宽度为(4.93±0.44)mm,外半规管骨岛宽度为(3.94±0.46)mm,前庭长度为(5.99±0.30)mm,前庭宽度为(3.08±0.19)mm,耳蜗底转长度为(8.47±0.39)mm,耳蜗底转宽度为(1.83±0.12)mm,上底转长度为(6.01±0.28)mm,中转长度为(4.15±0.27)mm,轴位耳蜗高度为(3.98±0.23)mm,冠位耳蜗高度为(5.10±0.37)mm,冠位耳蜗宽度为(6.68±0.40)mm。不同性别及左右耳间内耳各测量值比较差异均未见统计学意义(P均>0.05)。结论规范和详细的内耳测量数据可以为准确的内耳畸形诊断提供依据。Objective To investigate the method and clinical application value of high-resolution computed tomography(CT)of the temporal bone in measuring various anatomical structures of the inner ear in normal hearing individuals.Methods The high-resolution CT image data of 100 patients(200 ears)with normal hearing who visited the Ear Department of the First Affiliated Hospital of Zhengzhou University from 2019 to 2021 were retrospectively analyzed.The bony island width of superior semicircular canal(SSCC),bony island width of posterior semicircular canal(PSCC),bony island width of lateral semicircular canal(LSCC),the length and width of the vestibule,the basal turn length and lumen of cochlea,the upper turn length of cochlea,the middle turn length of cochlea and the cochlea height were all measured on the axial plane.The cochlea height and width were measurse from the coronal plane.The differences of various measurement indicators were compared between patients with different genders and between left and right ears.Results High resolution CT scans of all subjects showed no structural abnormalities in the inner and middle ears,no visible or linear vestibular aqueduct,and a midpoint width of less than 1.5 mm.The normal range of measurements for various structures in the inner ear were as follows:the bony island width of SSCC was(5.71±0.36)mm,the bony island width of PSCC was(4.93±0.44)mm,the bony island width of LSCC was(3.94±0.46)mm,the vestibule length and width were(5.99±0.30)mm and(3.08±0.19)mm,respectively,the basal turn length and lumen of cochlea were(8.47±0.39)mm and(1.83±0.12)mm,respectively,the upper turn length of cochlea was(6.01±0.28)mm,the middle turn length of cochlea was(4.15±0.27)mm,the axial cochlea height was(3.98±0.23)mm,the coronal cochlea height and width were(5.10±0.37)mm and(6.68±0.40)mm,respectively.There was no statistically significant difference in the measurement values of the inner ear between patients with different genders and between the left and right ears(all P>0.05).Conclusion
