肺力咳合剂联合氨溴索注射液及哌拉西林他唑巴坦在老年肺炎治疗中的应用价值

摘　　要：目的探讨肺力咳合剂联合氨溴索注射液及哌拉西林他唑巴坦在老年肺炎治疗中的治疗效果。方法队列研究。抽取2022年1月至2023年12月洛阳市中心医院收治的老年肺炎患者78例,按随机数字表法分为对照组与研究组,每组39例。对照组采用氨溴索注射液联合哌拉西林他唑巴坦治疗,研究组在对照组的治疗基础上加用肺力咳合剂。比较两组临床疗效、症状消失时间及不良反应发生率。比较两组治疗前后炎症因子[C反应蛋白(CRP)、白细胞介素-6(IL-6)、降钙素原(PCT)]水平和肺功能[最大呼气峰流速(PEF)、用力肺活量(FVC)]。结果研究组总有效率(94.87%,37/39)高于对照组(71.79%,28/39),P<0.05。研究组咳嗽、咳痰、肺部啰音消失时间短于对照组,差异有统计学意义(P<0.05)。治疗后,研究组CRP、IL-6、PCT水平低于对照组(P<0.05),研究组PEF、FVC水平高于对照组(P<0.05)。研究组不良反应发生率(10.26%,4/39)与对照组(5.13%,2/39)比较差异未见统计学意义(P>0.05)。结论肺力咳合剂联合氨溴索注射液和哌拉西林他唑巴坦治疗老年肺炎效果显著,能减轻机体肺部炎症,促使咳嗽、咳痰、肺部啰音等症状消失,改善患者肺功能,且治疗安全性良好。Objective To investigate the application value of Feilike mixture combined with ambroxol injection and piperacillin-tazobactam in the treatment of senile pneumonia.Methods Seventy-eight elderly patients with pneumonia admitted to Luoyang Central Hospital from January 2022 to December 2023 were selected for the cohort study,and they were divided into a control group and a study group according to random number table method,with 39 cases in each group.The control group was treated with ambroxol injection combined with piperacillin-tazobactam,and the study group was treated with Feilike mixture based on the treatment of the control group.The clinical efficacy,symptoms resolution time and incidence of adverse reactions were compared between the two groups.The levels of inflammatory factors,including C-reactive protein(CRP),interleukin-6(IL-6)and procalcitonin(PCT),and lung function assessed by maximum expiratory peak flow(PEF)and forced vital capacity(FVC)were compared between the two groups before and after treatment.Results The total effective rate of the study group(94.87%,37/39)was higher than that of the control group(71.79%,28/39),P<0.05.Compared with the control group,the time of symptoms resolution(cough,sputum,pulmonary rales)of the study group was shorter(P<0.05).After treatment,the levels of CRP,IL-6 and PCT in the study group were lower,and the levels of PEF and FVC were higher compared with the control group(P<0.05).There was no significant difference in the incidence of adverse reactions between the study group(10.26%,4/39)and the control group(5.13%,2/39),P>0.05.Conclusions Feilike mixture combined with ambroxol injection and piperacillin-tazobactam is effective in the treatment of senile pneumonia,which can reduce lung inflammation,and promote the disappearance of symptoms,such as cough,sputum and pulmonary rales;moreover,it can improve lung function,with good therapeutic safety.
