复方鳖甲软肝片联合富马酸丙酚替诺福韦片治疗慢性乙肝肝硬化失代偿期患者的效果观察

摘　　要：目的探讨在慢性乙肝肝硬化失代偿期患者中联合应用复方鳖甲软肝片与富马酸丙酚替诺福韦片治疗的干预价值。方法队列研究。抽取2020年1月至2023年12月晋城市人民医院收治的86例慢性乙肝肝硬化失代偿期患者,按随机数字表法分为对照组(43例,富马酸丙酚替诺福韦片)和观察组(43例,复方鳖甲软肝片联合富马酸丙酚替诺福韦片)。比较两组治疗效果,治疗前后肝功能指标、肝纤维化指标,不良反应发生情况。结果观察组治疗有效率(95.35%,41/43)高于对照组(81.40%,35/43),P<0.05。治疗前,两组肝功能指标、肝纤维化指标比较差异未见统计学意义(P>0.05);治疗后,两组白蛋白水平均升高,总胆红素、天冬氨酸转氨酶、丙氨酸转氨酶、透明质酸、层粘蛋白水平均降低,且观察组变化幅度大于对照组(P<0.05)。两组不良反应发生率比较差异未见统计学意义(P>0.05)。结论慢性乙肝肝硬化失代偿期患者中应用复方鳖甲软肝片联合富马酸丙酚替诺福韦片治疗,可提升治疗效果,改善肝功能指标与肝纤维化指标,安全可靠。Objective To investigate the intervention value of compound Biejia Ruangan tablets combined with tenofovir alafenamide fumarate tablets in the treatment of chronic hepatitis B cirrhosis in decompensation stage.Methods Eighty-six patients with chronic hepatitis B cirrhosis in decompensation stage admitted to Jincheng People’s Hospital from January 2020 to December 2023 were selected for the cohort study,and they were divided into a control group(43 cases,treated with tenofovir alafenamide fumarate tablets)and a study group(43 cases,treated with compound Biejia Ruangan tablets combined with tenofovir alafenamide fumarate tablets).The therapeutic effects,preoperative and postoperative liver function indexes,preoperative and postoperative hepatic fibrosis indexes and incidence of complications were compared between the two groups.Results The effective rate of treatment in the observation group(95.35%,41/43)was higher than that in the control group(81.40%,35/43),P<0.05.There was no significant difference in the levels of liver function indexes and liver fibrosis indexes between the two groups before treatment(P>0.05).After treatment,albumin level in the two groups increased,in addition,the levels of total bilirubin,aspartate transaminase,alanine transaminase,hyaluronic acid and laminin in the two groups decreased,moreover,the change in the observation group was greater than that in the control group(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusions Compound Biejia Ruangan tablets combined with tenofovir alafenamide fumarate tablets in the treatment of chronic hepatitis B cirrhosis in decompensation stage can improve the therapeutic effect,improve the liver function indicators and liver fibrosis indicators,which is safe and reliable.
